摘  要:目的:分析中性粒细胞与淋巴细胞比值(NLR)、乳酸脱氢酶(LDH)、D-二聚体(DDR)在儿童肺炎支原体坏死性肺炎(MPNP)评估诊断中的应用价值。方法:选取2022年6月-2024年6月泉州市妇幼保健院·儿童医院收治的82例肺炎支原体肺炎(MPP)患儿作为研究对象,根据其是否发生坏死性肺炎(NP)分为MPNP组(39例)和非MPNP组(43例)。比较两组基线资料和实验室检查结果,使用logistic逐步回归分析MPP患儿发生MPNP的影响因素,绘制受试者工作特征曲线,以曲线下面积(AUC)评价各影响因素对MPP患儿发生MPNP的预测效能。结果:两组NLR、LDH及DDR水平比较,差异有统计学意义(P<0.05)。logistic回归分析结果显示,NLR、LDH及DDR水平为MPP患儿发生MPNP的影响因素(P<0.05)。NLR、LDH及DDR水平预测MPP患儿发生MPNP的AUC分别为0.759、0.691、0.732,三者联合预测的AUC为0.858,高于单独检测,差异有统计学意义(P<0.001)。联合检测的敏感度为69.23%,特异度为95.35%。结论:NLR、LDH及DDR水平与MPP患儿发生MPNP呈明显相关性,可作为患儿发生MPNP的预测指标,联合检测可提高诊断准确性。Objective:To analyze the application value of neutrophil to lymphocyte ratio(NLR),lactate dehydrogenase(LDH)and D-dimer(DDR)in evaluation and diagnosis of mycoplasma pneumoniae necrotizing pneumonia(MPNP)in children.Method:A total of 82mycoplasma pneumoniae pneumonia(MPP)children admitted to Quanzhou Maternal and Child Health Hospital and Children's Hospital from June 2022 to June 2024 were selected as the study objects.According to whether they developed necrotic pneumonia(NP),they were divided into MPNP group(39 cases)and non-MPNP group(43 cases).Baseline data and laboratory test results were compared between the two groups,and the influencing factors of MPNP in children with MPP were analyzed by logistic stepstep regression.The working characteristic curve of subjects was drawn,and the area under the curve(AUC)was used to evaluate the predictive efficacy of each influencing factor in MPNP in children with MPP.Result:There were significant differences in NLR,LDH and DDR levels between the two groups(P<0.05).logistic regression analysis showed that NLR,LDH and DDR levels were the influencing factors for MPNP in MPP children(P<0.05).The AUC predicted by NLR,LDH and DDR levels for MPNP in MPP children was 0.759,0.691 and 0.732,respectively.The AUC predicted by NLR,LDH and DDR levels was 0.858,which was higher than that by single detection,and the difference was statistically significant(P<0.001).The sensitivity and specificity of combined detection were 69.23%and 95.35%.Conclusion:NLR,LDH and DDR levels were significantly correlated with MPNP in children with MPP,and could be used as predictors of MPNP in children with MPP.Combined detection could improve the diagnostic accuracy.
