摘　　要：目的:探讨通窍活血汤加减治疗急性脑梗死(ACI)的效果及对炎症和脂蛋白相关磷脂酶A2(Lp-PLA2)的影响。方法:选取2022年2月-2024年1月溧阳市中医医院神经内科收治并被确诊为ACI的120例患者纳入研究对象,按照电脑随机数字表法划分为对照组和观察组,每组各60例。对照组以常规西药进行治疗,观察组在对照组基础上辅以通窍活血汤治疗。比较两组实验室指标(血清和炎症因子水平)、临床疗效,以及不良反应。结果:两组治疗前血清、炎症因子和中医症候积分均比较,差异无统计学意义(P>0.05),治疗后两组各项指标较治疗前降低且观察组比对照组低,且观察组疗效高于对照组,差异有统计学意义(P<0.05);两组不良反应比较,差异无统计学意义(P>0.05)。结论:应用通窍活血汤加减与西药结合治疗急性脑梗死效果显著,可缓解临床症状,促进血液循环,降低炎症反应,治疗效果显著。Objective:To explore the effect of modified Tongqiao Huoxue decoction in the treatment of acute cerebral infarction and its influence on inflammation and lipoprotein-associated phospholipase A2(Lp-PLA2).Method:A total of 120 patients diagnosed with ACI admitted to the Department of Neurology,Liyang Hospital of Chinese Medicine from February 2022 to January 2024 were included in the study,and were divided into control group and observation group according to computer random number table method,with 60 cases in each group.The control group was treated with conventional western medicine,while the observation group was treated with Tongqi Huoxue decoction on the basis of the control group.Laboratory indicators(serum and inflammatory factor levels),clinical efficacy,and adverse reactions were compared between the two groups.Result:There was no difference in serum,inflammatory factors,and TCM symptom scores before treatment between the two groups(P>0.05).After treatment,all indicators in the two groups were lower than those before treatment,and the indicators in the observation group were lower than those in the control group(P<0.05).The curative effect of the traditional Chinese medicine group was higher than that of the western medicine group(P<0.05).There was no difference in adverse reactions between the two groups(P>0.05).Conclusion:The application of Tongqi Huoxue decoction combined with Western medicine in the treatment of acute cerebral infarction has a remarkable effect,which can alleviate clinical symptoms,promote blood circulation,reduce inflammation,and have a remarkable therapeutic effect.
