锚钉修复三角韧带联合常规钢板螺钉内固定术治疗踝关节骨折患者的临床效果及对踝关节恢复情况的影响

摘　　要：目的探究锚钉修复三角韧带联合常规钢板螺钉内固定术治疗踝关节骨折患者的临床效果及对踝关节恢复情况、内踝间隙的影响。方法回顾性选取2019年1月至2023年12月连江县总医院骨科收治的80例踝关节骨折患者的临床资料,根据治疗方法不同分为对照组和观察组,每组40例。对照组的治疗方法为常规钢板螺钉内固定术,观察组在对照组的基础上联合实施锚钉修复三角韧带治疗。比较两组踝关节恢复情况、疼痛程度、内踝间隙。结果术前,两组踝关节功能评定表评分、视觉模拟评分法(Visual Analogue Scale,VAS)评分、内踝间隙对比,差异均无统计学意义(P均>0.05)。术后1个月、术后2个月、术后3个月,观察组踝关节功能评定量表评分高于对照组,差异均有统计学意义(P均<0.05);观察组VAS评分分别为(4.18±0.64)分、(1.94±0.37)分、(0.84±0.25)分,低于对照组的(4.67±0.68)分、(2.26±0.43)分、(1.03±0.29)分,差异均有统计学意义(t=3.319,3.568,3.138;P均<0.05);观察组内踝间隙小于对照组,差异有统计学意义(P<0.05)。结论对踝关节骨折患者实施锚钉修复三角韧带联合常规钢板螺钉内固定术治疗可以减轻其疼痛程度,减小内踝间隙,提高踝关节修复效果。Objective To explore the clinical effect of anchor repair of deltoid ligament combined with conventional plate screw internal fixation in the treatment of ankle fracture and its influence on ankle joint recovery and medial clear space.Methods The clinical data of 80 patients with ankle fracture admitted to the Department of Orthopaedics in Lianjiang County General Hospital from January 2019 to December 2023 were retrospectively selected and divided into control group and observation group according to different treatment methods,with 40 cases in each group.The treatment method of the control group was conventional plate screw internal fixation,and the observation group was treated with anchor repair of deltoid ligament on the basis of the control group.The ankle joint recovery,pain degree and medial clear space were compared between the two groups.Results Before operation,there were no significant dif-ferences in American Othopaedic Foot and Ankle Society(AOFAS)score,Visual Analogue Scale(VAS)score and me-dial clear space between the two groups(all P>0.05).At 1 month,2 months and 3 months after operation,the ankle function rating scale score of the observation group were higher than those of the control group,and the differences were statistically significant(all P<0.05).The VAS scores of the observation group were(4.18±0.64)points,(1.94±0.37)points,(0.84±0.25)points,which were lower than those of the control group(4.67±0.68)points,(2.26±0.43)points,(1.03±0.29)points,the differences were statistically significant(t=3.319,3.568,3.138;all P<0.05).The inter clear space in the observation group were smaller than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Anchor repair of deltoid ligament combined with conventional plate screw internal fixation for patients with ankle fracture can reduce the degree of pain,reduce the medial malleolus gap,and improve the effect of ankle joint repair.
