髂筋膜阻滞对老年髋部骨折术后疼痛程度及认知功能的影响

摘　　要：目的分析髂筋膜阻滞对老年髋部骨折术后疼痛程度及认知功能的影响。方法便利选择2022年1月至2023年12月启东市第三人民医院收治的102例髋部骨折患者为研究对象,根据不同镇痛方法分为对照组(n=51)、研究组(n=51)。两组均行手术治疗,对照组采用药物镇痛,研究组结合术中髂筋膜阻滞。对比两组视觉模拟评分法(Visual Analogue Scale,VAS)评分、蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)评分、生活质量简表(World Health Organization Quality of Life-100,WHOQOL-100)评分。结果术后5 h、10 h和20 h,研究组VAS评分均低于对照组,差异均有统计学意义(P均<0.05)。术后1 d,研究组MoCA评分为(18.58±2.07)分,高于对照组的(16.46±2.15)分,差异有统计学意义(t=5.073,P<0.05)。术后1个月,研究组WHOQOL-100评分高于对照组,差异有统计学意义(P<0.05)。结论行髋部骨折术的老年患者实施髂筋膜阻滞,能够缓解术后疼痛,促进认知功能恢复,提高生活质量。Objective To analyze the effect of fascia iliac block on postoperative pain and cognitive function in elderly patients with hip fracture.Methods A total of 102 patients with hip fracture admitted to Qidong Third People's Hospi-tal from January 2022 to December 2023 were conveniently selected as the research objects.According to different an-algesic methods,they were divided into control group(n=51)and study group(n=51).Both groups were treated with surgery.The control group was treated with drug analgesia,and the study group was combined with intraoperative iliac fascia block.The Visual Analogue Scale(VAS)score,Montreal Cognitive Assessment(MoCA)score and World Health Organization Quality of Life-100(WHOQOL-100)score were compared between the two groups.Results At 5 h,10 h and 20 h after operation,the VAS scores of the study group were lower than those of the control group,and the differ-ences were statistically significant(all P<0.05).One day after operation,the MoCA score of the study group was(18.58±2.07)points,which was higher than that of the control group(16.46±2.15)points,and the difference was statis-tically significant(t=5.073,P<0.05).One month after operation,the WHOQOL-100 score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The imple-mentation of fascia iliac block in elderly patients undergoing hip fracture surgery can relieve postoperative pain,pro-mote the recovery of cognitive function and improve the quality of life.
