经皮肾镜手术术中体温过低采用综合保温护理的效果及价值

摘　　要：目的探讨经皮肾镜手术术中体温过低采用综合保温护理的效果。方法随机选取2021年1月-2023年1月泰安市中医医院收治的120例经皮肾镜手术患者为研究对象。依据不同护理方法分为对照组(60例)采用常规护理,观察组(60例)在对照组基础上采用综合保温护理,对比两组患者不同时间点的体温、心率(heart rate,HR)、平均动脉压(mean arterial pressure,MAP)及体温过低、寒战、躁动的发生率。结果术前,两组患者体温水平比较,差异无统计学意义(P>0.05);手术30 min、手术60 min、术毕、术毕60 min,观察组体温水平均高于对照组,差异均有统计学意义(P均<0.05)。术前,两组患者HR、MAP比较,差异均无统计学意义(P均>0.05);手术30 min、手术60 min、术毕、术毕60 min,两组患者HR、MAP均高于术前,但观察组各时间点HR、MAP低于对照组,差异均有统计学意义(P均<0.05)。观察组体温过低、寒战、躁动发生率分别为6.67%(4/60)、6.67%(4/60)、1.67%(1/60),均低于对照组的23.33%(14/60)、20.00%(12/60)、18.33%(11/60),差异均有统计学意义(χ^(2)=6.536,4.615,9.259;P均<0.05)。结论综合保温护理模式应用于经皮肾镜手术患者,可有效预防术中体温过低等不良情况的发生,维持循环稳定性。Objective To explore the effect of comprehensive insulation nursing for hypothermia during percutaneous nephroscopic surgery.Methods 120 patients with percutaneous nephrolithotomy treated in Tai'an Hospital of Tradi-tional Chinese Medicine from January 2021 to January 2023 were randomly selected as the study objects.According to different nursing methods,the control group(60 cases)was divided into conventional nursing,and the observation group(60 cases)was given comprehensive insulation nursing on the basis of the control group.The body temperature,heart rate(HR),mean arterial pressure(MAP)at different time points and the incidence of hypothermia,chills and agi-tation were compared between the two groups.Results Before operation,there was no significant difference in body temperature between the two groups(P>0.05).The body temperatures of the observation group were higher than those of the control group at 30 min,60 min,after operation and 60 min after operation,and the differences were statisti-cally significant(all P<0.05).Before surgery,there were no significant differences in HR and MAP between the two groups(both P>0.05).At 30 min,60 min,after operation and 60 min after operation,HR and MAP in two groups were higher than before operation,but HR and MAP in observation group were lower than control group at each time point,the differences were statistically significant(all P<0.05).The incidence rates of hypothermia,chills and agitation in the observation group were 6.67%(4/60),6.67%(4/60)and 1.67%(1/60),respectively,which were lower than those in the control group 23.33%(14/60),20.00%(12/60)and 18.33%(11/60),and the differences were statistically signifi-cant(χ^(2)=6.536,4.615,9.259;all P<0.05).Conclusion The application of comprehensive insulation nursing mode in percutaneous nephroscopic surgery can effectively prevent the occurrence of hypothermia and maintain circulation sta-bility.
