锁骨钩钢板治疗对肩锁关节脱位及锁骨远端骨折患者血清炎症因子、肩关节功能的影响

摘　　要：目的 探究锁骨钩钢板在肩锁关节脱位(acromio clavicular dislocation,ACD)及锁骨远端骨折(distal clavicle fracture,DCF)患者中的应用效果。方法 选取2018年1月—2024年1月东营市东营区人民医院收治的80例ACD及DCF患者作为研究对象,根据不同治疗方法分为两组。对照组(n=40)给予克氏针固定治疗,观察组(n=40)给予锁骨钩钢板治疗。比较两组临床疗效、手术相关指标、血清炎症因子、肩关节功能评分量表(Constant-Murley Shoulder Score,Constant-Murley)评分及并发症发生情况。结果 相较于对照组,观察组治疗总有效率较高,手术时间、骨折愈合时间、住院时间均较短,差异均有统计学意义(P均<0.05);治疗后,观察组肿瘤坏死因子-ɑ水平(9.01±1.32)pg/L、C反应蛋白水平(24.47±6.42)mg/L及白细胞介素-1水平(0.23±0.04)μg/mL均低于对照组的(12.51±1.68)pg/L、(56.78±7.34)mg/L、(0.26±0.04)μg/mL,差异均有统计学意义(t=10.361,20.955,3.354;P均<0.05);且治疗后1个月、3个月及6个月,观察组Constant-Murley评分均较高,差异均有统计学意义(P均<0.05)。观察组并发症发生率为2.50%(1/40),低于对照组的20.00%(8/40),差异有统计学意义(χ^(2)=4.507,P<0.05)。结论 对ACD及DCF患者应用锁骨钩钢板治疗,可减轻机体炎症反应,促进肩关节功能恢复,降低并发症发生率。Objective To investigate the application effect of clavicular hook plate in patients with acromio clavicle dislocation(ACD)and distal clavicle fracture(DCF).Methods 80 patients with ACD and DCF treated in Dongying District People´s Hospital from January 2018 to January 2024 were selected as study objects and divided into two groups according to different treatment methods.The control group(n=40)was given Kirschner needle fixation treatment,and the observation group(n=40)was given clavicular hook plate treatment.The clinical efficacy,surgical related indexes,serum inflammatory factors,Constant-Murley Shoulder Score(Constant-Murley)scores and complications were compared between the two groups.Results Compared with the control group,the total effective rate of the observation group was higher,and the operation time,fracture healing time and hospital stay were shorter,the differences were statistically significant(all P<0.05).After treatment,The levels of tumor necrosis factor-α(9.01±1.32)pg/L,C-reactive protein(24.47±6.42)mg/L and interleukin-1(0.23±0.04)μg/mL in the observation group were all lower than those in the control group(12.51±1.68)pg/L,(56.78±7.34)mg/L,(0.26±0.04)μg/mL,the differences were statistically significa(t=10.361,20.955,3.354;all P<0.05);At 1 month,3 months and 6 months after treatment,the Constant-Murley score of the observation group was higher,the differences were statistically significant(all P<0.05).The incidence of complications in the observation group was 2.50%(1/40)lower than that in the control group 20.00%(8/40),the difference was statistically significant(χ^(2)=4.507,P<0.05).Conclusion Clavicular hook plate can reduce the inflammatory reaction,promote the recovery of shoulder joint function and reduce the incidence of complications in patients with ACD and DCF.
