负压封闭引流技术对下肢开放性骨折术后感染患者血清炎性因子及创面疼痛的影响

摘　　要：目的探讨下肢开放性骨折术后感染患者采用负压封闭引流技术(vacuum sealing drainage,VSD)对血清炎性因子及创面疼痛的影响。方法随机选取2020年1月—2023年1月仁怀市中医院收治的60例下肢开放性骨折术后感染患者为研究对象,根据不同治疗方法分成研究组(n=30)与对照组(n=30),对照组给予常规引流技术治疗,研究组给予VSD治疗。比较两组血清炎性因子指标、疼痛程度、肉芽组织覆盖率、细菌清除率。结果两组C-反应蛋白、肿瘤坏死因子-α、白细胞介素-6、降钙素原、白细胞介素-8水平治疗前差异均无统计学意义(P均>0.05)。相较于治疗前,两组血清炎性因子水平、疼痛程度评分治疗后均更低,且相比于对照组,研究组均更低,差异均有统计学意义(P均<0.05)。治疗前,两组患者的肉芽组织覆盖率比较,差异无统计学意义(P>0.05)。相较于治疗前,两组治疗后的肉芽组织覆盖率更高,研究组比对照组更高,差异均有统计学意义(P均<0.05)。细菌清除率两组对比,研究组较对照组更高[(95.58±4.09)%比(91.79±5.63)%],差异有统计学意义(t=2.983,P<0.05)。结论下肢开放性骨折术后感染患者采用VSD治疗,能够促进炎症反应缓解,减轻疼痛,促进伤口肉芽组织生长,并有效清除细菌。Objective To explore the influence of vacuum sealing drainage(VSD)technology on serum inflammatory factors and wound pain in patients with postoperative infection of open fractures of the lower limbs.Methods A total of 60 patients with postoperative infection of open fractures of the lower limbs who were admitted to Renhuai City Traditional Chinese Medicine Hospital from January 2020 to January 2023 were randomly selected as the research objects.According to different treatment methods,they were divided into the research group(n=30)and the control group(n=30).The control group was treated with conventional drainage technology,while the research group was treated with VSD.The indexes of serum inflammatory factors,pain degree,granulation tissue coverage rate and bacterial clearance rate were compared between the two groups.Results There were no significant differences in the levels of C-reactive protein,tumor necrosis factor-α,interleukin-6,procalcitonin and interleukin-8 between the two groups before treatment(all P>0.05).Compared with before treatment,the levels of serum inflammatory factors and pain scores of the two groups were lower after treatment,and compared with the control group,the study group was lower,the differences were statistically significant(all P<0.05).Before treatment,there was no significant difference in granulation tissue coverage between the two groups(P>0.05).Compared with before treatment,the granulation tissue coverage rate of the two groups after treatment was higher,and the study group was higher than the control group,the differences were statistically significant(all P<0.05).The bacterial clearance rate of the study group was higher than that of the control group[(95.58±4.09)%vs(91.79±5.63)%],and the difference was statistically significant(t=2.983,P<0.05).Conclusion For patients with postoperative infection of open fractures of the lower limbs,the application of VSD treatment can promote the remission of inflammatory reactions,relieve pain,promote the growth of granulation tissue
