真武汤联合血府逐瘀方加减治疗老年冠状动脉粥样硬化性心脏病并慢性心力衰竭的临床研究

摘　　要：目的探究真武汤联合血府逐瘀方加减治疗老年冠状动脉粥样硬化性心脏病(coronary atherosclerotic heart disease,CHD)并慢性心力衰竭(chronic heart failure,CHF)的临床效果。方法选取2021年12月—2023年12月北京市第一中西医结合医院收治的88例老年CHD并CHF患者为研究对象,以不同治疗方法分两组,各44例。对照组予以稳心颗粒治疗,基于此,观察组予以真武汤+血府逐瘀方加减治疗。对比两组中医证候积分、心功能指标、心衰标志物水平、不良反应。结果治疗后,观察组气短(1.24±0.15)分、喘促(1.17±0.13)分、肢冷(1.31±0.29)分、心悸(1.05±0.11)分,均较对照组的(2.01±0.23)分、(1.95±0.26)分、(2.12±0.43)分、(1.89±0.13)分低,差异有统计学意义(t=18.601,17.799,10.359,32.720;P均<0.05)。观察组治疗后左室舒张末期内径、左室收缩末期内径和脑钠肽均较对照组低,左室射血分数较对照组高,差异有统计学意义(P均<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论真武汤联合血府逐瘀方加减治疗CHD并CHF患者可减轻临床症状,调节心衰标志物水平,改善心功能,且安全可靠。Objective To explore the clinical effect of Zhenwu Decoction combined with modified Xuefu Zhuyu Decoction in the treatment of elderly patients with coronary atherosclerotic heart disease(CHD)and chronic heart failure(CHF).Methods From December 2021 to December 2023,88 elderly patients with CHD and CHF admitted to Beijing First Integrated Traditional Chinese and Western Medicine Hospital were selected as the research objects.They were divided into two groups according to different treatment methods,44 cases in each group.The control group was treated with Wenxin Granules.Based on this,the observation group was treated with Zhenwu Decoction+Xuefu Zhuyu Decoction.The traditional Chinese medicine syndrome score,cardiac function index,heart failure marker level and adverse reactions were compared between the two groups.Results After treatment,the scores of shortness of breath(1.24±0.15)points,shortness of breath(1.17±0.13)points,cold limbs(1.31±0.29)points and palpitation(1.05±0.11)points in the observation group were lower than those in the control group(2.01±0.23)points,(1.95±0.26)points,(2.12±0.43)points and(1.89±0.13)points,and the differences were statistically significant(t=18.601,17.799,10.359,32.720;all P<0.05).After treatment,the left ventricular end diastolic diameter,left ventricular end systolic diameter and brain natriuretic peptide in the observation group were lower than those in the control group,and the left ventricular ejection fraction was higher than that in the control group the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Zhenwu Decoction combined with modified Xuefu Zhuyu Decoction in the treatment of CHD and CHF patients can reduce clinical symptoms,regulate the level of heart failure markers,improve cardiac function,and is safe and reliable.
