肠内营养联合双歧杆菌三联活菌胶囊预防脓毒症机械通气患者发生呼吸机相关性肺炎的效果

摘　　要：目的 分析肠内营养联合双歧杆菌三联活菌胶囊预防脓毒症机械通气患者发生呼吸机相关性肺炎(ventilator-associated pneumonia, VAP)的效果。方法 方便选取2021年1月—2023年12月福建省三明市第二医院收治的102例脓毒症机械通气患者作为研究对象,根据不同治疗方法,将患者分为对照组(肠内营养)和研究组(肠内营养+双歧杆菌三联活菌胶囊),各51例。对比两组临床疗效、免疫指标和炎症因子[白细胞介素-6(interleukin-6, IL-6)、血清降钙素原(procalcitonin, PCT)、C-反应蛋白(C-reactive protein, CRP)]水平。结果治疗后,研究组VAP发生率7.84%(4/51),低于对照组的23.53%(12/51),差异有统计学意义(χ^(2)=4.744,P<0.05)。两组菌血症、病死率比较,差异均无统计学意义(P均>0.05)。治疗后,研究组机械通气时间、ICU住院时间、IL-6、CRP、PCT、免疫指标水平均优于对照组,差异均有统计学意义(P均<0.05)。结论 肠内营养联合双歧杆菌三联活菌胶囊能够有效降低机械通气患者VAP的发生率,缩短机械通气时间和ICU住院时间,同时改善患者的炎症状态和免疫功能。Objective To analyze the effect of enteral nutrition combined with Bifidobacterium triple viable capsules in preventing ventilator-associated pneumonia(VAP)in septic patients undergoing mechanical ventilation.Methods A total of 102 patients with sepsis mechanical ventilation admitted to the Second Hospital of Sanming from January 2021 to December 2023 were selected as the research objects.According to different treatment schemes,the patients were divided into control group(enteral nutrition)and study group(enteral nutrition+bifidobacterium triple viable capsule),with 51 cases in each group.The clinical efficacy,immune indexes and levels of inflammatory factor[inter-leukin-6(IL-6),serum procalcitonin(PCT)and C-reactive protein(CRP)]were compared between the two groups.Results After treatment,the incidence of VAP in the study group was 7.84%(4/51),which was lower than that in the control group 23.53%(12/51),and the difference was statistically significant(χ^(2)=4.744,P<0.05).There was no signifi-cant difference in bacteremia and mortality between the two groups(both P>0.05).After treatment,the mechanical ventilation time,ICU hospitalization time,IL-6,CRP,PCT and immune indexes of the study group were better than those of the control group,and the differences were statistically significant(all P<0.05).Conclusion Enteral nutrition combined with bifidobacterium triple viable capsules can effectively reduce the incidence of VAP in patients undergo-ing mechanical ventilation,shorten the mechanical ventilation time and ICU hospitalization time,and simultaneously improve the inflammatory state and immune function of patients.
