小剂量阿司匹林联合低分子肝素钠在子痫前期患者中的应用价值

摘　　要：目的探究小剂量阿司匹林联合低分子肝素钠(low molecular weight heparin sodium,LMWHS)在子痫前期患者中的应用价值研究。方法将2021年1月—2023年12月期间,苏州市第九人民医院接收的90例子痫前期患者作为研究对象,根据治疗方式分为研究组(n=45)和对照组(n=45),对照组给予小剂量阿司匹林肠溶片,研究组在对照组基础上给予LMWHS皮下注射,两组均治疗两周。对比两组患者治疗前后的血压、24 h尿蛋白定量、凝血指标以及妊娠结局。结果治疗后,研究组血压、24 h尿蛋白定量、D-二聚体及纤维蛋白原低于对照组,活化部分凝血活酶时间和凝血酶原时间高于对照组,差异均有统计学意义(P均<0.05)。研究组的胎盘早剥发生率[0(0/45)]低于对照组[8.89%(4/45)],差异有统计学意义(χ^(2)=4.186,P<0.05)。结论联合使用低剂量阿司匹林和LMWHS能显著稳定子痫前期患者的血压和减少蛋白尿,改善其凝血功能和妊娠结局,在临床上具有借鉴意义。Objective To explore the application value of low-dose aspirin combined with low molecular weight heparin sodium(LMWHS)in patients with preeclampsia.Method From January 2021 to December 2023,90 patients with preeclampsia received by Suzhou Ninth People's Hospital were divided into study group(n=45)and control group(n=45)according to the treatment method.The control group was given low-dose aspirin enteric-coated tablets,and the study group was given LMWHS subcutaneous injection on the basis of the control group.Both groups were treated for two weeks.The blood pressure,24 h urine protein quantification,coagulation index and pregnancy outcome were compared between the two groups before and after treatment.Results After treatment,the blood pressure,24 h urine protein quantification,D-dimer and fibrinogen in the study group were lower than those in the control group,and the activated partial thromboplastin time and prothrombin time were higher than those in the control group,the differences were statistically significant(all P<0.05).The incidence of placental abruption in the study group[0(0/45)]was lower than that in the control group[8.89%(4/45)],and the difference was statistically significant(χ^(2)=4.186,P<0.05).Conclusion The combined use of low-dose aspirin and LMWHS can significantly stabilize blood pressure and reduce proteinuria in patients with preeclampsia,and improve their coagulation function and pregnancy outcome,which has clinical reference significance.
