含吲哚美辛硅橡胶宫内节育器与屈螺酮炔雌醇片在人工流产术后避孕中的效果比较

摘　　要：目的比较含吲哚美辛硅橡胶宫内节育器与屈螺酮炔雌醇片在人工流产术后避孕中的效果。方法回顾性选取2022年1月—2023年1月在浦城县妇幼保健院实施人工流产术的168例患者的临床资料,根据不同术后干预方法分为节育器组与药物组,每组84例,节育器组在患者术后立即放置含吲哚美辛硅橡胶宫内节育器,药物组在患者术后当日开始服用屈螺酮炔雌醇片。比较两组人工流产术后恢复情况,术后6个月、12个月避孕失败情况,计划外停用情况,不良反应发生率及避孕满意度。结果节育器组术后1个月子宫内膜厚度低于药物组,术后出血时间及术后月经恢复时间长于药物组,差异均有统计学意义(P均<0.05)。术后12个月,节育器组避孕失败率及计划外停用率分别为1.19%(1/84)和2.38%(2/84),低于药物组的9.52%(8/84)、11.90%(10/84),差异均有统计学意义(χ^(2)=4.226,5.744;P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。节育器组避孕满意度高于药物组,差异有统计学意义(P<0.05)。结论屈螺酮炔雌醇片有利于人工流产术后恢复,而含吲哚美辛硅橡胶宫内节育器能提高人工流产术后避孕效果,减少计划外停用,且患者满意度更高,两种避孕方式均具有较高安全性。Objective To compare the effect of indomethacin-containing silicone rubber intrauterine device and dro⁃spirenone ethinyl estradiol tablets in contraception after artificial abortion.Methods The clinical data of 168 patients who underwent artificial abortion in Pucheng County Maternal and Child Health Hospital from January 2022 to Janu⁃ary 2023 were retrospectively selected.Different postoperative intervention methods were divided into intrauterine de⁃vice group and drug group,with 84 cases in each group.The intrauterine device group was placed with indomethacincontaining silicone rubber intrauterine device immediately after operation,and the drug group began to take drospire⁃none ethinyl estradiol tablets on the day after operation.The recovery after artificial abortion,contraceptive failure at 6 months and 12 months after operation,unplanned discontinuation,the incidence of adverse reactions and contracep⁃tive satisfaction were compared between the two groups.Results The endometrial thickness at 1 month after surgery of the intrauterine device group was lower than that of the drug group,and the postoperative bleeding time and postopera⁃tive menstrual recovery time were longer than those of the drug group,the differences were statistically significant(all P<0.05).At 12 months after operation,the contraceptive failure rate and unplanned discontinuation rate in the intra⁃uterine device group were 1.19%(1/84)and 2.38%(2/84),respectively,which were lower than 9.52%(8/84)and 11.90%(10/84)in the drug group,and the differences were statistically significant(χ^(2)=4.226,5.744;both P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The contra⁃ceptive satisfaction of the intrauterine device group was higher than that of the drug group,and the difference was sta⁃tistically significant(P<0.05).Conclusion Drospirenone and ethinyl estradiol tablets are beneficial to the recovery afterartificial abortion,while indomethacin-containing silicone rubbe
