腹腔镜下卵巢打孔术联合戊酸雌二醇治疗多囊卵巢综合征合并不孕症患者的临床疗效

摘　　要：【目的】探讨腹腔镜下卵巢打孔术(LOD)联合戊酸雌二醇治疗多囊卵巢综合征(PCOS)合并不孕症患者的临床疗效。【方法】选取2019年6月至2022年4月本院收治的117例PCOS合并不孕症患者,按治疗方法不同将患者分为对照组(采取LOD联合克罗米芬治疗,n=56)和观察组(采取LOD联合戊酸雌二醇治疗,n=61),两组均治疗3个月经周期,比较两组临床疗效。【结果】治疗后,观察组促卵泡刺激素(FSH)、雌二醇(E_(2))、卵泡数目、子宫内膜厚度均高于对照组,促黄体生成素(LH)水平低于对照组(P<0.05)。观察组治疗总有效率、优质卵泡率、随访1年妊娠率高于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。【结论】采用LOD联合戊酸雌二醇治疗PCOS合并不孕症患者,可显著患者提高优质卵泡率和妊娠率,改善患者卵巢功能和子宫内膜厚度,且并不会增加不良反应发生率。【Objective】To investigate the clinical effect of laparoscopic ovariectomy(LOD)combined with estradiol valerate in the treatment of polycystic ovary syndrome(PCOS)patients with infertility.【Methods】A total of 117 infertility patients with PCOS admitted to our hospital from June 2019 to April 2022 were recruited.According to different treatment methods,patients were divided into two groups.The control group(n=56)was treated with LOD combined with clomiphene,and the observation group(n=61)was treated with LOD combined with estradiol valerate.Patients in both groups were treated for 3 menstrual cycles,and the efficacy of the two groups was compared.【Results】After treatment,follicle stimulating hormone(FSH),estradiol(E_(2)),follicle number and endometrial thickness in the observation group were higher than those in control group,while luteinizing hormone(LH)in the obseation group was lower than that in control group(P<0.05).The total treatment effective rate,high-quality follicle rate and 1-year follow-up pregnancy rate in the observation group were higher than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).【Conclusion】LOD combined with estradiol valerate for treating PCOS patients with infertility can significantly improve the quality follicle rate and pregnancy rate,improve ovarian function and endometrial thickness,and not increase the incidence of adverse reactions.
