郭氏便血方治疗湿热下注型内痔便血临床研究

摘　　要：目的:观察郭氏便血方治疗湿热下注型内痔便血的临床疗效。方法:选取2022年10月—2023年12月浙江中医药大学附属嘉兴中医院收治的80例湿热下注型内痔便血患者,按照随机数字表法分为对照组和观察组各40例,研究期间无剔除和脱落。对照组采用马应龙麝香痔疮膏治疗,观察组采用郭氏便血方治疗。比较2组临床疗效。治疗前后,比较2组中医证候积分及血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。比较2组血管内皮生长因子(VEGF)、成纤维细胞生长因子2(FGF2)水平,Western Blot检测Rho相关蛋白激酶A(RhoA)、Rho相关卷曲螺旋蛋白激酶(ROCK)1、ROCK2表达水平。结果:观察组总有效率92.50%(37/40),高于对照组72.50%(29/40)(P<0.05)。治疗后,2组便血、小便短赤、肛门坠胀评分和血清IL-6、TNF-α水平均较治疗前降低(P<0.05),观察组便血、小便短赤、肛门坠胀评分和血清IL-6、TNF-α水平均低于对照组(P<0.05)。2组内痔组织中VEGF、FGF2、RhoA、ROCK1、ROCK2表达水平均低于对照组(P<0.05)。结论:郭氏便血方可有效缓解湿热下注型内痔便血患者的临床症状,减少炎症反应,提高临床疗效。Objective:To observe the clinical effects of GUO's Bianxue Prescription in treating internal hemorrhoids with hematochezia of damp-heat pouring downward type.Methods:A total of 80 patients with internal hemorrhoids with hematochezia of damp-heat pouring downward type admitted to Jiaxing Traditional Chinese Medicine Hospital Affiliated to Zhejiang Chinese Medical University from October 2022 to December 2023 were selected and randomly divided into a control group and an observation group according to the random number table method,with 40 cases in each group.No cases were excluded or lost during the study period.The control group was treated with Mayinglong Musk Hemorrhoids Ointment,while the observation group was treated with GUO's Bianxue Prescription.The clinical efficacy of the two groups was compared.Before and after treatment,the traditional Chinese medicine syndrome scores and serum levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were compared between the two groups.The expression levels of vascular endothelial growth factor(VEGF)and fibroblast growth factor 2(FGF2)were compared between the two groups.Western Blot detected the protein expression levels of Rho-associated protein kinase A(RhoA),Rho-associated coiled-coil protein kinase(ROCK)1,and ROCK2.Results:The total effective rate in the observation group was 92.50%(37/40),which was higher than that of 72.50%(29/40)in the control group(P<0.05).After treatment,the scores for hematochezia,short and dark urine,and sagging distension of the anal,as well as the serum levels of IL-6 and TNF-α,were significantly decreased in both groups compared to those before treatment(P<0.05).The scores for hematochezia,short and dark urine,and sagging distension of the anal,and the serum levels of IL-6 and TNF-αin the observation group were lower than those in the control group(P<0.05).The expression levels of VEGF and FGF2,and the protein expression levels of RhoA,ROCK1,and ROCK2 in the internal hemorrhoid tissues were lower in the observation group than
