摘 要:目的使用层次分析法(AHP)及逼近理想解排序法(TOPSIS)对普胸外科手术预防用抗菌药物的合理性进行评价,为标准化开展抗菌药物专项点评工作并促进其合理使用提供参考。方法建立普胸外科手术预防性使用抗菌药物的合理用药评价标准。选取淮北市人民医院2022年9月至2023年3月进行普胸外科手术并预防用抗菌药物的病历为干预前组,2023年4月至10月进行手术并预防用药病历为干预后组,采用AHP加权TOPSIS法评价围术期预防用抗菌药物合理性,比较干预前后用药合理率。结果干预前组Ci≥0.6的病历42例,占比73.7%,Ci<0.6的病历15例,占比26.3%;干预后组Ci≥0.6的病历52例,占比96.3%,Ci<0.6的病历2例,占比3.7%;干预前与干预后合理率比较差异有统计学意义(P<0.01),品种选择比较差异有统计学意义(P<0.01),术后用药比较差异有统计学意义(P<0.05)。结论采用AHP加权TOPSIS法评价围术期预防性抗菌药物用药合理性,可提高药师点评的同质化,并在加强预防用抗菌药物干预管理、标准化点评方面提供参考。Objective To evaluate the rationality of antibiotics for prevention in general thoracic surgery,the Analytic Hierarchy Process(AHP)and the TOPSIS were used to provide reference for standardizing the spe-cial evaluation of antibiotics and promoting their rational use.Methods To establish rational drug use evaluation criteria for prophylactic use of antibiotics in general thoracic surgery.The medical records of general thoracic surgery and prophylactic use of antibiotics from September 2022 to March 2023 were selected as the pre-interven-tion group,and the medical records of surgery and prophylactic use from April to October 2023 were selected as the post-intervention group.The rationality of perioperative prophylactic use of antibiotics was evaluated by AHP weighted TOPSIS method,and the rational rate of drug use before and after the intervention was compared.Re-sults Before intervention,42 cases(73.7%)had Ci≥0.6,and 15 cases(26.3%)had Ci<0.6.After intervention,52 cases had Ci≥0.6,accounting for 96.3%,and 2 cases had Ci<0.6,accounting for 3.7%.There were statistical-ly significant differences in the rationality rate before and after intervention(P<0.01),the selection of varieties had statistically significant differences(P<0.01),and the postoperative drug use had statistically significant differences(P<0.05).Conclusion The use of AHP weighted TOPSIS method to evaluate the rationality of perioperative pro-phylactic antibiotics can improve the homogeneity of pharmacist reviews and provide reference for strengthening the intervention management and standardized reviews of prophylactic antibiotics.
