摘　　要：目的分析医院病区药房针剂药品破损的原因,寻找合理有效的防治措施,以降低针剂药品破损率,减少医院经济损失、资源浪费以及减少危险药品的污染。方法选定降低病区药房针剂破损率为活动主题,通过PDCA循环法的计划、执行、检查、处置4个阶段对主题进行改进。2023年3~10月每月记录破损针剂药品的药名、规格、数量和破损原因,分析原因,采取有效措施防治破损,对比干预前后的药品破损情况。结果应用PDCA循环法,病区药房针剂破损率由改善前的0.0263%降低至0.0102%,相对于目标值0.0103%,目标达标率100.63%,进步率为61.22%,破损金额同比下降70.6%。结论开展防治措施后,药品破损率有所控制,降低了药品资源浪费,PDCA循环法可节约医院经济成本,保障患者用药安全,同时大幅度提高药学人员工作责任心和自我管理能力。Objective To analyze the causes of injectables breakage in Fifth Affiliated Hospital of Wen-zhou Medical University′s pharmacy,to find reasonable and effective preventive measures,in order to reduce the breakage rate,minimize the economic losses,prevent resource wastage,and reduce pollution from hazardous drugs.Methods With the theme of reducting the breakage rate of injectable medications in hospital pharmacy,im-provements were made through the four stages of the PDCA cycle(plan do,check and act).From March to October 2023,records were kept monthly of the name,specifications,quantity,and reasons of breakage for damaged in-jectables.The reasons were analyzed,and effective measures were implemented to prevent and control the break-age.The breakage rates were compared before and after the intervention.Results By applying the PDCA cycle,the breakage rate of injectables in the pharmacy decreased from 0.0263%before improvement to 0.0102%.Com-pared to the target value of 0.0103%,the target achievement rate was 100.63%,with a progress rate of 61.22%,and reducing breakage-related costs by 70.6%.After carrying out the preventive measures,the breakage rate was effectively controlled,avoiding resources waste.Conclusion PDCA Cycle significantly reduced economic costs for the hospital,ensured medication safety,and greatly improved the sense of responsibility and self-management ability of pharmacy personnel.
