柴胡旋覆花汤联合左甲状腺素钠片对桥本甲状腺炎患者的影响

摘　　要：目的探讨桥本甲状腺炎(Hashimoto's thyroiditis,HT)患者采用柴胡旋覆花汤联合左甲状腺素钠片治疗的效果。方法根据治疗方法的不同,将2020年11月—2023年11月莘县人民医院收治的78例HT患者分为两组。对照组(39例)给予左甲状腺素钠片治疗,中医组(39例)在对照组基础上联合柴胡旋覆花汤治疗。对比两组临床疗效、中医证候积分、甲状腺功能以及不良反应发生率。结果在治疗总有效率方面,中医组[94.87%(37/39)]高于对照组[71.79%(28/39)],差异有统计学意义(χ^(2)=7.477,P<0.05)。治疗后,与对照组相比,中医组中医证候积分均更低,游离三碘甲腺原氨酸、游离甲状腺素水平更高,促甲状腺激素、甲状腺球蛋白抗体水平更低,差异均有统计学意义(P均<0.05)。在不良反应发生率方面,组间比较,差异无统计学意义(P>0.05)。结论柴胡旋覆花汤联合左甲状腺素钠片能降低HT患者的中医证候积分,有效改善其甲状腺功能,疗效显著,安全性良好。Objective To investigate the effect of Chaihu Xuanfuhua Decoction combined with levothyrox-ine sodium tablets in the treatment of Hashimoto's thyroiditis(HT).Methods According to different treatment methods,78 patients with HT admitted to Shenxian People's Hospital from November 2020 to November 2023 were divided into two groups.The control group(39 cases)was treated with levothyroxine sodium tablets,and the traditional Chinese medicine group(39 cases)was treated with Chaihu Xuanfuhua Decoction on the basis of the control group.The clinical efficacy,traditional Chinese medicine syndrome scores,thyroid function and the incidence of adverse reactions were compared between the two groups.Results In terms of the total effective rate of treatment,the traditional Chinese medicine group[94.87%(37/39)]was higher than the control group[71.79%(28/39)],and the difference was statistically significant(χ^(2)=7.477,P<0.05).After treatment,compared with the control group,the traditional Chinese medicine syndrome scores of the traditional Chinese medicine group were lower,the levels of serum free triiodothyronine and free thyroxine were higher,the levels of thyroid stimulating hormone and thyroglobulin antibody were lower,the differences were statistically significant(all P<0.05).In terms of the incidence of adverse reactions,there was no significant difference between the two groups(P>0.05).Conclusion Chaihu Xuanfuhua Decoction combined with levothyroxine sodium tablets can reduce the traditional Chinese medicine syndrome scores of HT patients,and can effectively improve their thyroid func-tion.The curative effect is significant and the safety is good.
