腹部创伤定点超声在急诊腹部闭合性创伤诊断中应用的效果分析

摘　　要：目的研究腹部创伤定点超声在急诊腹部闭合性创伤诊断中的应用效果。方法选取2022年1—12月聊城市人民医院收治的80例疑似胸腹部闭合性创伤患者,均接受常规彩超检查、腹部创伤定点超声检查,以手术结果为金标准,分析不同检查方法的诊断效能。结果腹部创伤定点超声检查的准确度为98.75%(79/80)、灵敏度为96.15%(25/26)、特异度为100.00%(54/54),均高于常规彩超的86.25%(69/80)、73.07%(19/26)、92.59%(50/54),差异均有统计学意义(χ^(2)=9.009、5.318、4.154,P均<0.05)。常规超声、腹部创伤定点超声对出血、挫伤、破裂的诊断符合率比较,差异均无统计学意义(P均>0.05)。结论急救腹部闭合性创伤患者采用腹部创伤定点超声法检查时,能够快速知晓其是否患有腹部闭合性创伤,并做出有效评估,可为临床救治腹部闭合性创伤中诊疗方案的制订提供依据。Objective To study the application effect of fixed-point ultrasound of abdominal trauma in the diagnosis of emergency abdominal closed trauma.Methods A total of 80 patients with suspected closed thoracic and abdominal trauma admitted to Liaocheng People's Hospital from January to December 2022 were selected.All patients underwent routine color Doppler ultrasound examination and fixed-point ultrasound examination of abdominal trauma.The diagnostic efficacy of different examination methods was analyzed with the surgical re-sults as the gold standard.Results The accuracy,sensitivity and specificity of fixed-point ultrasound examina-tion for abdominal trauma were 98.75%(79/80),96.15%(25/26)and 100.00%(54/54),respectively,which were higher than 86.25%(69/80),73.07%(19/26),92.59%(50/54)of routine color Doppler ultrasound,and the dif-ferences were statistically significant(χ^(2)=9.009,5.318,4.154,all P<0.05).There were no significant differ-ences in the diagnostic coincidence rate of hemorrhage,contusion and rupture between routine color Doppler ul-trasound and fixed-point ultrasound examination for abdominal trauma(all P>0.05).Conclusion When pa-tients with emergency abdominal closed trauma are examined by fixed-point ultrasound of abdominal trauma,they can quickly know whether they have abdominal closed trauma and make effective evaluation,which can provide a basis for the formulation of diagnosis and treatment plan in clinical treatment of abdominal closed trauma.
