磁共振成像联合多层螺旋CT检查在膝关节隐匿性骨折诊断中的应用价值

摘　　要：目的分析磁共振成像(magnetic resonance imaging,MRI)、多层螺旋CT(multi-slice spiral CT,MSCT)联用对膝关节隐匿性骨折的诊断价值。方法选取2022年1月—2023年1月金沙县中医医院收治的60例疑似膝关节隐匿性骨折患者,所有患者均采取MRI、MSCT与关节镜检查,以关节镜或手术结果为金标准,对比MRI、MSCT与单独及联合检查的诊断效能及kappa值。结果经关节镜或手术结果确诊阳性43例,MRI检出阳性40例,MSCT检出阳性42例,MRI联合MSCT检出阳性42例。MRI联合MSCT的kappa值为0.798。MSCT联合MRI的准确度、灵敏度、特异度、阳性预测值、阴性预测值均高于MSCT、MRI单一检查,差异均有统计学意义(P均<0.05)。结论MRI联合MSCT对膝关节隐匿性骨折的诊断准确度效能较高,具有一定的临床应用价值。Objective To analyze the diagnostic value of magnetic resonance imaging(MRI)and multi-slice spiral CT(MSCT)in the diagnosis of occult fracture of knee joint.Methods A total of 60 patients with sus-pected occult knee fracture admitted to Jinsha County Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were selected.All patients were examined by MRI,MSCT and arthroscopy,with ar-throscopic or surgical results as the gold standard.The diagnostic efficacy and kappa value of MRI,MSCT alone and combined examination were compared.Results There were 43 positive cases confirmed by arthroscopic or surgical results,40 positive cases detected by MRI,42 positive cases detected by MSCT,and 42 positive cases detected by MRI combined with MSCT.The kappa value of MRI combined with MSCT was 0.798.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of MSCT combined with MRI were higher than those of MSCT and MRI alone,and the differences were statistically significant(all P<0.05).Conclusion MRI combined with MSCT has high diagnostic efficacy for occult fracture of knee joint,has certain application value.
