摘　　要：目的分析阴道彩色多普勒血流显像(color Doppler flow imaging,CDFI)联合促黄体生成素(luteinizing hormone,LH)/促卵泡生成素(follicle-stimulating hormone,FSH)、雌二醇(estradiol,E2)/孕酮(progesterone,P)在多囊卵巢综合征早期筛查中的应用价值。方法选取2022年4月—2023年12月东莞市第八人民医院收治的86例疑似多囊卵巢综合征患者为研究对象,以临床综合诊断为金标准,所有疑似患者均进行阴道CDFI、LH/FSH、E2/P检查,统计临床综合诊断、阴道CDFI、LH/FSH、E2/P检查对多囊卵巢综合征的诊断结果,比较阴道CDFI、LH/FSH、E2/P单独检查及三者联合检查对多囊卵巢综合征的诊断效能(灵敏度、特异度、准确度)以及阴性和阳性患者的LH/FSH、E2/P指标水平。结果经临床综合诊断多囊卵巢综合征阳性75例,阴性11例;阴道CDFI检查阳性73例,LH/FSH检查阳性77例,E2/P检查阳性74例,三者联合检查阳性74例。三者联合诊断多囊卵巢综合征的灵敏度、特异度均高于阴道CDFI、LH/FSH、E2/P单独诊断,差异均有统计学意义(P均<0.05)。阴性组的LH/FSH、E2/P水平为1.15±0.03、91.50±10.62、均优于阳性组的2.17±0.24、48.89±10.63,差异均有统计学意义(t=14.010、12.417,P均<0.05)。结论阴道CDFI联合LH/FSH、E2/P在多囊卵巢综合征早期筛查中的灵敏度较高。Objective To analyze the application value of transvaginal color doppler flow imaging(CDFI)combined with luteinizing hormone(LH)/follicle-stimulating hormone(FSH)and estradiol(E2)/progesterone(P)in the early screening of polycystic ovary syndrome.Methods 86 patients with suspected polycystic ovary syn-drome admitted to Dongguan Eighth People's Hospital from April 2022 to December 2023 were selected as the research objects.The clinical comprehensive diagnosis was the gold standard.All suspected patients were exam-ined by transvaginal CDFI,LH/FSH and E2/P.The results of clinical comprehensive diagnosis,transvaginal CDFI,LH/FSH and E2/P in the diagnosis of polycystic ovary syndrome were statistically analyzed.The diagnos-tic efficacy(sensitivity,specificity,accuracy)of transvaginal CDFI,LH/FSH,E2/P alone and the combined ex-amination of the three in the diagnosis of polycystic ovary syndrome,and the LH/FSH andE2/P indexes levels of negative and positive patients were compared.Results The clinical comprehensive diagnosis of polycystic ovary syndrome positive in 75 cases and negative in 11 cases,transvaginal CDFI examination was positive in 73 cases,LH/FSH examination was positive in 77 cases,E2/P examination was positive in 74 cases,and the combined ex-amination of the three was positive in 74 cases.The sensitivity and specificity of the combined diagnosis of poly-cystic ovary syndrome were higher than that of vaginal CDFI,LH/FSH and E2/P alone,and the difference were statistically significant(both P<0.05).The levels of LH/FSH and E2/P in the negative group were 1.15±0.03 and 91.50±10.62,which were better than 2.17±0.24 and 48.89±10.63 in the positive group,and the differences were statistically significant(t=14.010,12.417,both P<0.05).Conclusion Transvaginal CDFI combined with LH/FSH and E2/P has high sensitivity in early screening of polycystic ovary syndrome.
