乳腺增生性结节与乳腺癌鉴别诊断中超声检查及BI-RADS分级的应用价值研究

摘  要:目的研究乳腺增生性结节与乳腺癌鉴别诊断中超声检查及BI-RADS分级的应用价值。方法选择2020年6月—2023年6月在广东省中山市黄圃人民医院接受诊断的200例乳腺疾病患者。将病理结果作为金标准,分析超声检查及BI-RADS分级对于乳腺增生性结节与乳腺癌的诊断符合率,比较超声检查及BI-RADS分级的鉴别诊断结果及诊断效能值。结果超声检查及BI-RADS分级检出乳腺病变的真阳性54例,真阴性133例,假阳性5例,假阴性8例。超声检查及BI-RADS分级对于乳腺疾病诊断的灵敏度为87.10%,特异度为96.38%,准确度为93.50%,kappa值为0.846。超声检查及BI-RADS分级对于乳腺增生性结节的诊断符合率为96.38%(133/138),对乳腺癌的诊断符合率为87.10%(54/62)。结论通过超声及BI-RADS分级诊断乳腺疾病的诊断效能值较高。Objective To study the application value of ultrasonography and the BI-RADS classification in the differential diagnosis of breast hyperplastic nodules and breast cancer.Methods 200 cases of breast disease patients diagnosed in Huangpu People's hospital of Zhongshan ctiy from June 2020 to June 2023 were selected as the observation objects.Taking the pathological results as the gold standard,we compared the ultrasound findings and the BI-RADS classification in the differential diagnosis of breast disease,as well as the differential diagnosis results of ultrasound for 3-4 types of breast lesions,the overall diagnosis results of breast lesions,the diagnostic coincidence rate between ultrasound examination and the BI-RADS classification for breast prolif-erative nodules and breast cancer was analyzed,and the characteristics of ultrasound imaging in the differential diagnosis of different breast lesions was compared.Results Itshowed 54 true positive,133 true negative,5 false positive,and 8 false negative cases.The sensitivity for breast disease diagnosis of ultrasonography and the BI-RADS classification was 87.10%(54/62),specificity of 96.38%(133/138),and accuracy of 93.50%(187/200).The Kappa value was 0.846.The diagnostic coincidence rate for proliferative breast nodules of ultrasonography and the BI-RADS classification was 96.38%(133/138)and 87.10%(54/62).Conclusion The diagnostic efficacy of ultrasound and BI-RADS classification in the diagnosis of breast diseases is high.
