摘　　要：目的 探讨抗信号识别粒子(SRP)抗体阳性间质性肺炎(SRP-IP)患者肌炎抗体谱、免疫功能及肺功能变化。方法 采用方便抽样法选择2022年1月至2023年12月在开封市中心医院就诊的84例SRP-IP患者进行研究。分析不同滴度抗SRP抗体阳性患者肌炎抗体谱、免疫功能、肺功能以及与患者预后的关系。结果 低滴度抗SRP抗体阳性患者与高滴度抗SRP抗体阳性患者在年龄分布、咳嗽症状、白细胞计数、C-反应蛋白、乳酸脱氢酶、IgA、IgM、用力肺活量占预计值百分比、第1秒用力呼气容积占预计值百分比、抗黑色素分化相关基因5抗体、抗干燥综合征抗原A的52 KD的多肽条段抗体等方面比较,差异有统计学意义(P<0.05)。低滴度抗SRP抗体阳性患者疗效显著优于高滴度患者(P<0.05)。结论 SRP-IP患者易合并其他抗体,患者免疫功能、肺功能均降低,高滴度抗SRP抗体阳性患者短期预后较差,病死率高。Objective To investigate the changes of myositis antibody spectrum,immune function and lung function of patients with antisignal-recognition particle antibody positive-interstitial pneumonia(SRP-IP).Methods According to convenient sampling method,84 patients with SRP-IP in Kaifeng Central Hospital were enrolled from January 2022 to December 2023.The myositis antibody spectrum,immune function and lung function,and their relationship with prognosis of SRP-IP patients with different titer were analyzed.Results There were significant differences in age distribution,cough symptoms,white blood cell count,C-reactive protein,lactate dehydrogenase,IgA,IgM,forced vital capacity as a percentage of the predicted value,forced expiratory volume in 1 second as a percentage of the predicted value,antibody of anti-melanin differentiation gene 5 and polypeptide segment antibody of anti-Sjogren's syndrome antigen A's 52 KD between patients with low titer and those with high titer(P<0.05).The curative effect in patients with low titer of anti-SRP antibody positive was significantly better than that of patients with high titer(P<0.05).Conclusions SRP-IP patients are susceptible to other antibodies,and patients have decreased immune function and lung function.Patients with high titer anti-SRP antibody positive have poor short-term prognosis and high fatality rate.
