摘　　要：目的探讨早期声门上型喉癌患者水平喉部分切除术后发生感染的影响因素。方法选择郑州大学第一附属医院2021年1月至2023年12月择期行水平喉部分切除术的142例早期声门上型喉癌患者为研究对象,统计患者基线资料、实验室指标及术后感染发生情况,采用logistic回归分析早期声门上型喉癌患者水平喉部分切除术后感染发生的影响因素。结果142例早期声门上型喉癌患者水平喉部分切除术后未发生感染115例,发生感染27例,发生率为19.01%;发生组年龄、合并糖尿病占比、手术时间、使用呼吸机占比均高于未发生组,CD4^(+)、CD4^(+)/CD8^(+)、白蛋白水平均低于未发生组(P<0.05);经logistic回归分析结果显示,年龄、合并糖尿病、手术时间、CD4^(+)、CD4^(+)/CD8^(+)是早期声门上型喉癌患者喉部分切除术后发生感染的影响因素(P<0.05)。结论年龄、合并糖尿病、手术时间、CD4^(+)、CD4^(+)/CD8^(+)水平是早期声门上型喉癌患者水平喉部分切除术后发生感染的影响因素。Objective To investigate the influencing factors of infection after horizontal partial laryngectomy in patients with early supraglottic laryngeal carcinoma.Methods A total of 142 patients with early supraglottic laryngeal cancer who underwent elective horizontal partial laryngectomy in the the First Affiliated Hospital of Zhengzhou University from January 2021 to December 2023 were selected as the study subjects.Baseline data,laboratory indicators,and postoperative infection incidence were collected.Logistic regression was used to analyze the influencing factors of infection after horizontal partial laryngectomy in patients with early supraglottic laryngeal cancer.Results Among 142 patients with early supraglottic laryngeal cancer,115 cases had no infection after horizontal partial laryngectomy,and 27 cases had infection,with an incidence of 19.01%.The age,the proportion of diabetes,the operation time,the proportion of ventilator use in the occurrence group were higher than those in the non occurrence group,and the levels of CD4^(+),CD4^(+)/CD8^(+),and albumin were lower than those in the non occurrence group(P<0.05).Logistic regression analysis showed that age,diabetes,operation time,CD4^(+),CD4^(+)/CD8^(+)were the influencing factors of infection in patients with early supraglottic laryngeal cancer after partial laryngectomy(P<0.05).Conclusion Age,diabetes,operation time,CD4^(+),CD4^(+)/CD8^(+)levels are the influencing factors of infection after horizontal partial laryngectomy in patients with early supraglottic carcinoma.
