节段式乳房按摩联合按摩仪对产妇产后乳汁分泌、乳房胀痛及母乳喂养成功率的影响

摘　　要：目的探讨给予产后产妇节段式乳房按摩(SBM)联合乳房按摩仪的治疗方案所产生的临床效果,包括对产后乳汁分泌、乳房胀痛和母乳喂养成功率的影响。方法选取医院于2023年1—12月收治的98例产妇为样本对象,采用随机数字表法将其分为对照组和观察组,两组均为49例。对照组采用常规治疗联合SBM,观察组在对照组的基础上采用乳房按摩仪进行治疗。观察并对比两组产妇干预前和干预后不同时间(1、2、3 d)的乳汁分泌情况、催乳素水平和乳房胀痛情况(VAS),对比两组产妇的乳房恢复情况(乳瘀斑块消失时间、乳房硬结消退时间和泌乳始动时间)和母乳喂养成功率。结果观察组产妇干预后的乳汁分泌量与催乳素水平高于对照组(P<0.05);观察组产妇干预后的VAS分数低于对照组(P<0.05);观察组产妇的泌乳始动时间、乳房硬结消退时间和乳瘀斑块消失时间更短(P<0.05);观察组产妇的母乳喂养成功率高于对照组(P<0.05)。结论SBM联合按摩仪可改善产妇产后的乳汁分泌情况,表现为乳汁分泌量和催乳素量持续提升,泌乳始动时间缩短,亦可改善产妇乳房胀痛情况,加速乳房硬结和斑块消退,并可提升母乳喂养成功率。Objective To explore the clinical effects of the treatment programme of segmental breast massage(SBM)combined with breast massager for postpartum mothers,including the effects on postpartum milk secretion,breast distension and breastfeeding success rate.Methods Ninety-eight cases of labouring mothers admitted to the hospital between January 2023 and December 2023 were selected as sample subjects,and were divided into a control group and an observation group using the random number table method,with 49 cases in both groups.The control group was treated with conventional treatment combined with SBM,and the observation group was treated with breast massager on the basis of the control group.Observe and compare the breast milk secretion,lactogen amount and breast distension[visual analogue scale(VAS)]of the two groups of mothers at different times(1,2,3 d)before and after the intervention,and compare the breast recovery(time of disappearance of breast stasis plaque,time of regression of breast hardness and time of initiation of lactation)and the success rate of breastfeeding of the mothers in the two groups.Results The amount of milk secretion and lactogen in the observation group was consistently higher than that in the control group(P<0.05).The VAS scores in the observation group were consistently lower than those in the control group(P<0.05).The time of initiation of lactation,the time of regression of breast nodules,and the time of disappearance of breast stasis were shorter in the observation group(P<0.05).The success rate of breastfeeding in the observation group was higher than that in the control group(P<0.05).Conclusion SBM combined with massage instrument can improve breast milk secretion after delivery,which can be manifested in the continuous improvement of milk secretion and lactogen quantity,the shortening of initiation time of lactation,the improvement of breast distension and pain,the acceleration of breast nodule and plaque disappearance,and the improvement of breastfeeding success rate.
