个案管理师主导的基于HCRM的多学科管理在不孕患者管理中的探索及实践

摘　　要：目的探讨个案管理师参与主导的多学科团队(MDT)基于医院客户关系管理随访系统(HCRM)的延续全流程管理随访模式在不孕患者中的探索及应用效果。方法选择2023年1—6月收治的不孕患者为观察组,实施以个案管理师为主导的依托HCRM的MDT延续全流程管理随访模式。回顾性观察2022年1—6月未进行个案管理的不孕患者为对照组。对两组患者分别在开单日和验孕日评估患者的焦虑状态和生育压力,统计建档率和建档用时及来院次数等指标进行比较分析。结果观察组患者的1个月经周期检查完成度、按时建档率、6个月建档率均高于对照组,平均建档用时、来院次数少于对照组,且观察组患者对护理服务的满意度更高,差异有统计学意义(P<0.05)。开单当天两组患者焦虑评分、生育压力比较差异无统计学意义(P>0.05)。经个案管理师主导MDT全流程管理随访干预后,观察组验孕日焦虑、生育压力均低于对照组,差异有统计学意义(P<0.05)。结论个案管理师为主导依托HCRM的MDT延续全流程管理随访模式适用于不孕患者,能够促进患者诊疗进展、缩短建档用时、减少非必要就医次数,缓解患者心理、经济压力,提高医疗资源利用率。Objective To investigate the case manager to participate in leading multi-disciplinary team(MDT)based on the hospital customer relationship management system(HCRM)the continuation of the whole process follow-up follow-up management mode in the exploration and application effect of infertility.Methods A total of 2023 patients with infertility admitted from January to June 2023 were selected as the observation group and were subjected to an MDT continuity of care management model with a case manager as the lead and HCRM as the support.The infertile patients without case management from January to June 2022 were retrospectively observed as the control group.The anxiety and fertility pressure of the two groups of patients on the day of prescription and pregnancy test were compared,and the rate and time of filing and the number of visits to the hospital were counted.Results The examination completion rate in one menstrual cycle,file construction rate on time,and file construction rate in six months in the observation group were higher than those in the control group,the average file establishment time and the number of visits to the hospital were less than those in the control group,and the patients in the observation group were more satisfied with the nursing service,and the differences were statistically significant(P<0.05).There was no significant difference in anxiety score and fertility pressure between the two groups on the day of physical examination(P>0.05).After the intervention of case management,the anxiety and fertility pressure of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The MDT continuous whole process management and follow-up mode based on HCRM led by case managers is suitable for infertility patients,which can promote the progress of diagnosis and treatment,shorten the time of filing,reduce the number of unnecessary medical visits,relieve the psychological and economic pressure of patients,and improve the util
