摘 要:髋关节发育不良(developmental dysplasia of the hip,DDH)以髋关节形态异常、髋臼发育不良或髋关节完全脱位为特征,由于解剖异常,髋关节常受到不同程度的接触应力,导致髋关节不稳定、髋关节生物力学异常,最终导致退行性关节炎。虽然全髋关节置换术(total hip arthroplasty,THA)已被证明是治疗DDH继发骨关节炎的有效方法,但由于病理解剖的原因,尤其对于Crowe Ⅲ~Ⅳ型DDH患者,术中髋关节重建对骨科医师来说在技术上具有挑战性。近年来,3D打印快速发展,它能显著帮助骨科医师制订手术方案,为患者提供最佳的治疗方案。笔者主要对Crowe Ⅲ~Ⅳ型DDH患者行THA髋臼侧和股骨侧的挑战以及3D打印技术的应用进展作一综述。Total hip arthroplasty(THA)for patients with development dysplasia of the hip(DDH)is a great challenge for the operator.Its deformed anatomical structure is accompanied by severe bone defect,which leads to difficulties in locating the acetabular cup,selecting and reducing the prosthesis.The 1:1 surgical model generated by 3D printing technology can clearly and intuitively display the complex anatomical structure of the acetabulum,which plays an important role in THA preoperative planning.3D printed personalized guide plate provides a new idea for the difficulty of locating the acetabular cup.The design of personalized acetabular prosthesis provides a new method for the complex anatomical structure of patients.This article mainly reviews the challenges of THA in acetabular and femoral sides of Crowe Ⅲ-Ⅳ DDH patients and the application progress of 3D printing technology.
