摘 要:目的 探讨物联网监护系统在重症急性胰腺炎(SAP)早期复苏容量管理中的应用效果。方法通过对接生命监护系统、智能尿流量监护系统、医嘱执行终端及重症护理记录系统,构建ICU物联网监护系统,并应用于SAP患者的早期容量管理。比较系统应用前(2021年1—12月)及应用后(2022年6月至2023年5月)各30例患者24 h及48 h液体复苏达标率、72 h并发症发生率及平均住院时间,同时比较系统应用前后,相关指标统计耗时及科室医护人员对系统的满意度情况。结果 系统应用后与应用前相比,患者24 h液体复苏达标率(60.00%vs 30.00%)、48 h液体复苏达标率(93.33%vs 46.67%)有所提高,72 h并发症发生率(23.33%vs 53.33%)及平均住院时间[(9.50±2.21)d vs (11.53±4.06)d]有所降低,相关指标统计平均耗时为(2.87±1.28)min,少于之前的(8.90±1.47)min;两组比较,差异均有统计学意义(P<0.05)。科室40名医护人员对于系统整体满意度较高,各维度评分从高到低依次为:系统操作简便性、促进团队协作、液体管理准确性及预警功能。结论 物联网监护系统有利于提高SAP患者早期液体复苏达标率、缩短患者住院时间,同时可提高医护人员的容量管理时效性。Objective To evaluate the application of IoT monitoring system in early resuscitation capacity management for patients with severe acute pancreatitis(SAP).Methods An ICU IoT monitoring system was developed by integrating a vital signs monitoring system,an intelligent urinary flow monitoring system,a medical order execution terminal and a critical care record system.The system was applied to the early capacity management of SAP patients.The following indicators were assessed in 30 patients and compared before(January–December 2021) and after(June 2022–May 2023) the application of the system:24-hour and 48-hour fluid resuscitation compliance rates,72-hour complication incidence and the average length of stay.Additionally,time spent on indicator statistics and the satisfaction of healthcare providers were compared before and after application.Results After application,the 24-hour fluid resuscitation compliance rate(60.00%vs.30.00%),the 48-hour fluid resuscitation compliance rate(93.33%vs.46.67%),the 72-hour complication incidence(23.33%vs.53.33%) and the average length of stay [(9.50±2.21)d vs.(11.53±4.06)d] were all better compared to those before application.The average time spent on indicator statistics decreased from(8.90±1.47) min to(2.87±1.28) min.The differences between the two groups were statistically significant(P<0.05).The 40 healthcare providers in the department expressed high satisfaction with the system as a whole.The dimensions were ranked from highest to lowest scores as follows:ease of system operation,promotion of teamwork,accuracy of fluid management and early warning function.Conclusion The IoT monitoring system enhances early fluid resuscitation compliance rate,shortens hospital stays and improves the timeliness of capacity management in SAP patients.
