中药单体调控铁死亡防治糖尿病肾病的研究进展

摘  要:糖尿病肾病(DN)是常见的糖尿病微血管并发症,也是终末期肾脏病的主要病因。DN以肾功能进行性下降、蛋白尿为主要表现,其发病机制复杂,目前尚无特异性防治措施。铁死亡是由于铁离子和过氧化脂质过度积累,氧化还原不平衡导致的一种调节性细胞死亡形式。在DN进展中,可见肾小管上皮细胞、足细胞、肾小球系膜细胞等发生铁死亡,抑制铁死亡可达到治疗DN的效果。中药单体因能直接作用于靶细胞、调控铁死亡靶点而备受关注,在DN临床防治方面潜力巨大。现将从铁死亡与DN的关系、中药单体抑制铁死亡治疗DN的作用机制等方面作一综述,以期为DN临床防治和新型药物开发提供新思路和新方向。Diabetic nephropathy(DN)is a common microvascular complication of diabetes mellitus and a major cause of end-stage renal disease.DN is characterised by progressive decline in renal function and proteinuria,and its pathogenesis is complex,with no specific preventive or curative measures available.Ferroptosis is a regulated form of cell death due to excessive accumulation of iron ions and lipid peroxide and redox imbalance.In the process of DN,ferroptosis can be seen in renal tubular epithelial cells,podocytes,glomerular mesangial cells,etc.Inhibiting Ferroptosis can achieve the effect of treating DN.Traditional Chinese medicine(TCM)monomers have attracted much attention for their ability to act directly on target cells and regulate ferroptosis targets,and they have great potential in the clinical prevention and treatment of DN.In this paper,we will review the relationship between ferroptosis and DN,and the mechanism of inhibition of ferroptosis in the treatment of DN by TCM monomers,with a view to providing new ideas and directions for the clinical prevention and treatment of DN and the development of novel drugs.
