摘 要:目的本研究采用欧洲脊柱体模(European spine phantom,ESP)检验GE-Lunar和Hologic两种不同品牌的双能X线吸收测量仪(dual-energy X-ray absorptiometry,DXA)参考数据库标准化之后的T值一致性。目的是验证该标准化数据库规范不同品牌DXA设备T值的效果。方法体模验证实验使用同一个欧洲脊柱体模(European spine phantom,ESP,编号SN145),由GE-Lunar和Hologic两个品牌的DXA扫描仪各扫描10次,每次扫描结束后重新定位。对ESP 3个椎骨重新划定出6个不同的区域,选取相对应的区域进行测量,分别记录两台机器采用各自国人数据库和标准化数据库的相应T值,采用Pearson相关分析检验两设备应用两种数据库测量L_(1-4)、L_(1-3)、L_(2-4)的T值相关性。结果GE-Lunar iDXA和Hologic Horizon-W两款DXA设备测量ESP骨密度值,前者较后者平均高18.4%,T值也具有显著差异。两设备应用标准化数据库测量L_(1-4)、L_(1-3)、L_(2-4)不同骨密度下的T值,其差异无统计学意义。结论标准化数据库应用于GE-Lunar iDXA和Hologic Horizon-W腰椎扫描的T值一致性好,对同一患者行腰椎测量可获得一致的诊断结论,它的应用可促进国内骨质疏松症的诊疗规范。Objective The T-scores of two different brands of dual-energy X-ray absorptiometry(DXA),GE-Lunar and Hologic,were compared before and after standardization of reference databases using the European spine phantom(ESP)to verify the feasibility of the standardized database.Methods The ESP was scanned 10 times on both machines with repositioning after each scan.Six different regions were identified on three vertebral bodies of the ESP,and the corresponding regions were selected for measurement.T-scores using the standardized database and the Chinese reference database were recorded for each machine.Pearson correlation analysis was used to test the correlation of T-scores from both databases for L_(1-4),L_(1-3),and L_(2-4).Results The results showed that bone mineral density measured by GE-Lunar iDXA was on average 18.4%higher than that measured by Hologic Horizon-W.However,the T-scores for L_(1-4),L_(1-3),and L_(2-4)measured using the standardized database showed no significant difference at different levels of bone density.Conclusions The standardized database is suitable for assessing T-score consistency between GE-Lunar iDXA and Hologic Horizon-W lumbar spine scans,yielding consistent diagnostic conclusions of lumbar spine measurements on the same patient.Its application can promote the standardization of diagnosis and treatment of osteoporosis in China.
