摘 要:二尖瓣反流是最常见的心脏瓣膜疾病,经导管二尖瓣置换术(transcatheter mitral valve replacement,TMVR)为手术高危患者提供了新的解决方案。本文针对TMVR产品近十几年发展,对产品面临的主要问题以及临床现状等进行了综述。重点对Tendyne、Intrepid、SAPIEN M3、Cephea、Altavalve和Innovalve 6款典型产品的特点、设计演化和临床现状进行了详细综述,以便于为行业工程人员、科研人员及医务人员对产品进一步的优化提供思路。目前TMVR产品在确保稳定锚定的同时,如何进一步提高瓣膜适配性和兼容性、患者筛选成功率和临床安全性,降低瓣周漏、左室流出道梗阻风险、瓣膜输送直径和并发症是后续需要重点解决的问题。Mitral regurgitation is the most common heart valve disease,and transcatheter mitral valve replacement offers a new solution for patients at high risk of surgery.This paper reviewed the development of transcatheter mitral valve replacement(TMVR)products over the past decade,focusing on the main problems faced by the products and the current clinical situation.It provided a detailed overview of the characteristics,design evolution and clinical status of six typical products:Tendyne,Intrepid,SAPIEN M3,Cephea,Altavalve and Innovalve,and offered ideas for further optimization of the products for industry engineers,researchers and medical personnel.At present,while ensuring the stable anchoring of TMVR products,how to further improve the valve suitability and compatibility,patient screening success rate and clinical safety,and reduce the risk of perivalvular leakage,left ventricular outflow tract obstruction,valve delivery diameter and complications are key problems to be solved in the future.
