基于“和”法辨治胃食管反流病

摘　　要：王华宁教授认为胃食管反流病的发病以脏腑气机失和为关键,脾胃虚弱为本,痰饮、湿热、瘀血为标。其主要病机为胃失和降,气机上逆,病位在胃和食管,与五脏均相关,其治疗注重调畅气机即恢复脾升胃降、肝升胃降、肺胃通降、心肾相交的生理状态,重视脾胃虚弱之本。善用“和”法治疗本病,即调整气机升降失常之状态,调和人体寒热、虚实之偏颇。具体遣方用药上善用药对,寒药与热药共用,升药与降药并入,气分药与血分药兼施,临床疗效显著,具体运用如升脾降胃调气机、健运中州调寒热、疏肝和胃降逆气、清上温下调枢机。Professor Wang Hua-ning considered that the pathogenesis of acid reflux disease is based on the weakness of the spleen and stomach,phlegm,dampness-heat and blood stasis are the main symptoms and the key point is the disharmony of zang-fu qi.The general pathogenesis of the disease is the disharmony of descending of the stomach and the reverse of Qi.The location of the disease is in the stomach and esophagus,and it is related to the spleen and stomach,liver and gallbladder,lung and kidney.The treatment should pay attention to the recovery of the physiological state of spleen ascending and stomach descending,liver ascending and stomach descending,lung and stomach descending,heart and kidney intersecting and the weakness of the spleen and stomach.To use the method of“Harmony”to treat this disease,that is to say,recovering the abnormal state of Qi,adjusting the human body cold and heat,false and true to balance.Professor Wang Huaning is good at using medicine pairs when prescribing,such as:cold medicine and hot medicine combined,rising and falling medicines combined,Qi and blood medicines combined,and the clinical efficacy is significant.Specificall,Regulating Qi by elevating the spleen and lowering the stomach,regulating cold and heat by Jianyun Zhongzhou,lowering the adverse qi by soothing the liver and regulating the stomach,regulating the qi by clearing the upper heat and warming the lower cold.
