基于数据挖掘探讨许丰敏治疗肝肾气血亏虚型膝骨关节炎的用药规律

摘　　要：目的:通过数据挖掘技术总结江西省名中医许丰敏治疗肝肾气血亏虚型膝骨关节炎(KOA)的用药组方规律。方法:收集许丰敏治疗肝肾气血亏虚型KOA患者的临证医案,将所获得的方剂建立数据库,统一规范录入中医传承辅助平台(V2.5)进行数据相关分析。结果:共收集许丰敏治疗肝肾气血亏虚型KOA的有效处方62首,药物113味,常用的药物有牛膝、当归、独活、川芎、熟地黄等。用药药性以温性频次最高,其次为平性和寒性;药味以甘、辛、苦为主,归经以肝、肾、脾经为主。关联规则揭示常用的用药组合有牛膝与当归,牛膝与独活,川芎与牛膝,川芎与当归等,并通过聚类分析得到2个可能新处方。结论:许丰敏认为“肝肾气血亏虚,外感风寒湿邪”为KOA的根本病机,治疗过程中注重补益肝肾气血,辅以祛风除湿。Objective:To summarize the medication composition rules of XU Fengmin,a renowned traditional Chinese medicine practitioner in Jiangxi Province,in the treatment of knee osteoarthritis(KOA)with liver and kidney,Qi and blood deficiency based on data mining.Methods:Collected clinical medical records of XU Fengmin's treatment of KOA patients with liver and kidney,Qi and blood deficiency,established a database of the obtained formulas,and uniformly and normatively input them into the Traditional Chinese Medicine Inheritance Assistance Platform(V2.5)for data related analysis.Results:A total of 62 effective prescriptions and 113 drugs were collected from KOA patients with liver and kidney,Qi and blood deficiency type.Commonly used medicines include Achyranthes bidentata,Angelica sinensis,Heracleum hemsleyanum,Ligusticum chuanxiong,Rehmannia glutinosa,etc.The frequency of medication is highest in warm nature,followed by mild and cold nature;the medicinal taste is mainly sweet,spicy,and bitter,with the liver,kidney,and spleen meridians as the main meridians.Analysis of association rules revealed:a total of prescription pairs were obtained,including Achyranthes bidentata and Angelica sinensis,Achyranthes bidentata and Heracleum hemsleyanum,Ligusticum Chuanxiong and Achyranthes bidentata,Ligusticum Chuanxiong and Angelica sinensis.Two possible new prescriptions were obtained by cluster analysis.Conclusion:XU Fengmin believes that the fundamental pathogenesis of KOA is"deficiency of liver and kidney,Qi and blood,and external pathogenic factors such as wind,cold,and dampness".During the treatment process,emphasis is placed on nourishing liver and kidney,Qi and blood,supplemented by dispelling wind and dampness.
