凉血四物汤加减在儿童扁桃体低温等离子全切术后的应用分析

摘　　要：目的:观察凉血四物汤加减在儿童扁桃体低温等离子切除术后的临床应用疗效。方法:选取2021年6月—2023年6月在新余市中医院耳鼻咽喉科收治并实施了双侧扁桃体低温等离子全切术的患儿70例,随机分为对照组和观察组,各35例。对照组采用西医常规治疗,观察组采用西医常规治疗联合口服凉血四物汤加减。评估2组术后出血、疼痛、发热、咳嗽、伪膜脱落时间等情况。结果:2组患儿术后第3、7天咽部异物感、刺激性咳嗽、气道阻塞的症状体征评分及总积分较本组术后第1天均逐渐下降,口臭评分均逐渐上升,其中,除观察组术后第3天口臭积分、对照组术后第3天气道阻塞积分及总积分较本组术后第1天的差异无统计学意义(P>0.05),其余差异均有统计学意义(P<0.05)。与对照组比较,观察组术后第2~7天疼痛评分更低,术后第3、7天咽部异物感、刺激性咳嗽、口臭、气道阻塞等症状评分及总积分更低,术后发热发生率更低,伪膜脱落时间更短,差异均有统计学意义(P<0.05),出血发生率更低,但差异无统计学意义(P>0.05)。结论:凉血四物汤加减能有效减轻儿童扁桃体低温等离子全切术后的不适症状,降低并发症发生率,促进伤口早期愈合。Objective:To observe the clinical effect of modified Liangxue Siwu Decoction in children after low-temperature plasma tonsillectomy.Methods:From June 2021 to June 2023,70 children who were admitted to the Department of Otolaryngology of Xinyu Hospital of Traditional Chinese Medicine and underwent bilateral tonsil cryogenic plasma total resection were randomly divided into the control group and the observation group,with 35 cases in each group.The control group was treated with conventional Western medicine,and the observation group was treated with conventional treatment of Western medicine combined with oral administration of modified Liangxue Siwu Decoction.Postoperative bleeding,pain,fever,cough,pseudomembrane detachment time and so on were evaluated in the two groups.Results:In both groups,the symptom scores for pharyngeal foreign body sensation,irritating cough,airway obstruction,and total scores gradually decreased on postoperative days 3 and 7 compared to postoperative day 1 within the same group,while bad breath scores progressively increased.Notably,except for the bad breath score in the observation group on postoperative day 3 and the airway obstruction score and total score in the control group on postoperative day 3 which showed no statistically significant differences compared to postoperative day 1 within their respective groups(P>0.05),all other differences were statistically significant(P<0.05).Compared with the control group,the observation group had mild pain symptoms from the second day after surgery,mild symptoms such as pharyngeal foreign body sensation,irritating cough,bad breath,and airway obstruction on the third and seventh days after surgery.The incidence of postoperative fever was low,the time for pseudomembrane detachment was shortened,and the difference was statistically significant(P<0.05).The incidence of bleeding was low,but the difference was not statistically significant(P<0.05).Conclusion:Modified Liangxue Siwu Decoction can effectively relieve the postoperative symptoms of tons
