缺血性脑卒中芤脉的脉动时相面积研究

摘　　要：目的:研究缺血性脑卒中芤脉的脉动时相图面积参数及缺血性脑卒中芤脉特点,为缺血性脑卒中辨证提供依据。方法:应用脉诊采集仪采集51例缺血性脑卒中患者的左关脉脉波图,分别从浮、中、沉采取脉图参数A、As、Ad、As/A、As/Ad,t、t_(1)、t_(4)、t_(5)、t_(1)/t、t-t_(1)、t_(4)/t_(5)。结果:与正常组比较,缺血性脑卒中组浮取t、t_(1)、t_(4)、t_(5)值均显著延长(P<0.05),中取t、t_(1)、t_(4)、t_(1)/t、t/(t-t_(1))均显著延长(P<0.05),沉取t、t_(1)、t_(4)、t_(1)/t均显著延长(P<0.05),浮取A、As、As/A,中取A、As、As/A、As/Ad,沉取As/A、As/Ad均显著增大(P<0.05);浮取与沉取面积参数A、As、Ad的差值均显著增大(P<0.05),中取与沉取面积参数A、As的差值均显著增大(P<0.05),浮取与中取面积参数As-Ad的差值显著增大(P<0.05)。结论:缺血性脑卒中患者的脉波图芤脉特点,面积参数体现出浮大中空的虚大特点,也兼有“来盛”“去衰”的洪大特点,其虚大机理不但为精血亏虚,更是阳虚血瘀,脉浮为阳浮祛邪反应,其“来盛”为邪不泄而正气急。Objective:The purpose of this study is to provide objective basis for the differentiation of deficiency and excess in the pulse phase diagram and area parameters of patients with ischemic stroke on the meridian.Methods:A pulse diagnosis collection device was used to collect the left Guan pulse waveform of 51 patients with ischemic stroke.Pulse parameters A,As,Ad,As/A,As/Ad,t,t_(1),t_(4),t_(5),t_(1)/t,t-t_(1),t_(4)/t_(5)were taken from the floating,middle,and sinking points,respectively.Results:Compared with the normal control group,the floating values of t,t_(1),t_(4),t_(5)in the ischemic stroke group were significantly prolonged(P<0.05),while the middle values of t,t_(1),t_(4),t_(1)/t,t/(t-t_(1))were significantly prolonged(P<0.05),and the sinking values of t,t_(1),t_(4)and t_(1)/t were significantly prolonged(P<0.05).Floating values of A,As,As/A,middle values of A,As,As/A,As/Ad,and sinking values of As/A and As/Ad were significantly increased(P<0.05);The difference between the floating and sinking area parameters A,As and Ad significantly increased(P<0.05),while the difference between the middle and sinking area parameters A and As significantly increased(P<0.05),and the difference between the floating and middle area parameters As-Ad significantly increased(P<0.05).Conclusion:The pulse wave diagram of the ischemic stroke patient group reflects the characteristics of the secondary pulse,where the area parameter can reflect its floating and hollow characteristics,showing the characteristics of qi and blood deficiency,as well as the characteristics of blood stasis,as well as the pulse characteristics of"coming prosperity"and"qi decline".This provides a reference basis for the syndrome of qi deficiency and blood stasis in ischemic stroke,and supplements the research content on local ischemia of the secondary pulse.
