摘 要:目的:基于MAPK/ERK信号通路探讨蒸脐疗法干预腹泻型肠易激综合征(IBS-D)模型大鼠的作用机制。方法:取SD大鼠24只,随机分为正常组、模型组、西药组、蒸脐组,每组6只。除正常组外,其余各组均采用番泻叶联合慢性温和束缚应激加孤养法造模。造模成功后,模型组大鼠给予10 mL/(kg·d)生理盐水灌胃;西药组予2 mg/mL匹维溴铵混悬液灌胃,剂量为10 mL/(kg·d);蒸脐组采用中药蒸脐疗法进行干预;每组均干预14 d。正常组仅给予10m L/(kg·d)生理盐水灌胃。观察各组大鼠一般情况、体重、粪便干湿比重、Bristol及AWR评分;采用RT-q PCR检测结肠和海马ERK1、ERK2、c-Fos的m RNA表达,Western Blot检测结肠和海马ERK1/2、p-ERK1/2、c-Fos的蛋白表达,观察结肠组织病理学变化。结果:干预后,与模型组比较,蒸脐组和西药组一般状况明显好转,体重增加明显(P<0.05),且蒸脐组大鼠体重高于西药组(P<0.05)。与模型组比较,蒸脐组和西药组Bristol评分、粪便干湿比重及不同压力梯度下AWR评分均降低(P<0.05),且蒸脐组均显著低于西药组(P<0.05)。与模型组比较,各组大鼠结肠和海马ERK1 mRNA、ERK2 mRNA、ERK1/2蛋白表达均无明显差异(P>0.05),蒸脐组和西药组的结肠、海马c-Fos mRNAs以及西药组结肠c-Fos蛋白表达均降低(P<0.05),蒸脐组结肠c-Fos蛋白表达无明显差异(P>0.05);西药组和蒸脐组的海马c-Fos、p-ERK1/2以及结肠p-ERK1/2蛋白表达均降低(P<0.05),且蒸脐组均低于西药组(P<0.05)。各组大鼠结肠组织结构基本正常,无明显充血糜烂、水肿或炎症细胞浸润等病理改变。结论:蒸脐疗法可能通过调控MAPK/ERK通路,调节c-Fos、p-ERK1/2的表达,从而改善内脏高敏感性,达到治疗IBS-D的目的。Objective:To explore the mechanism of steaming navel therapy intervention in Diarrhea Predominant Irritable Bowel Syndrome(IBS-D)model rats based on the MAPK/ERK signaling pathway.Methods:24 SD rats were randomly divided into a normal group,a model group,a Western medicine group,and a steamed navel group,with 6 rats in each group.Except for the normal group,all other groups were modeled using a combination of senna leaves and chronic mild restraint stress plus solitary care.After successful modeling,the model group rats were given 10 mL/(kg·d)of physiological saline by gavage.The Western medicine group was given a 2 mg/mL suspension of povidone bromide by gavage at a dose of 10 mL/(kg·d).The steaming navel group was intervened with traditional Chinese medicine steaming navel therapy.Each group was intervened for 14 days.The normal group was only given 10 mL/(kg·d)of physiological saline by gavage.Observe the general condition of rats,including body weight,fecal dry wet density,Bristol and AWR scores.RT-qPCR was used to detect the mRNA expression of ERK1,ERK2,and c-Fos in the colon and hippocampus.Western blot was used to detect the protein expression of ERK1/2,p-ERK1/2,and c-Fos in the colon and hippocampus,and the pathological changes in colon tissue were observed.Results:After intervention,compared with the model group,the general condition of the steamed navel group and the Western medicine group improved significantly,with a significant increase in body weight(P<0.05),and the body weight of rats in the steamed navel group was higher than that in the Western medicine group(P<0.05).Compared with the model group,the Bristol score,fecal dry wet density,and AWR score under non pressure gradient were all reduced in the steamed navel group and the western medicine group(P<0.05),and the steamed navel group was significantly lower than the western medicine group(P<0.05).Compared with the model group,there was no significant difference in the expression of ERK1 mRNA,ERK2 mRNA,and ERK1/2 protein in the colon and hipp
