摘 要:目的比较经尿道低功率(60W)前列腺绿激光剜除术(GreenLEP)与高功率(160W)前列腺绿激光汽化术(PVP)治疗良性前列腺增生症的疗效与安全性。方法回顾性分析2021年1月至2022年12月110例行经尿道绿激光手术治疗前列腺增生患者的临床资料。GreenLEP组和PVP组各55例,比较两组手术时间、术后2 h血红蛋白丢失量、住院时间、尿管留置时间、术后3个月时排尿情况及手术并发症等指标。结果GreenLEP组和PVP组手术时间[(47.3±19.4)min VS.(52.5±22.4)min]、尿管保留时间[(3.4±0.5)d VS.(4.6±0.4)d]、术后住院时间[(4.4±0.5)d VS.(5.4±1.0)d],差异有统计学意义(P<0.05);术后2 h血红蛋白下降值(8.5±3.2)g/L VS.(7.8±1.9)g/L、暂时性尿失禁(3 VS.2)两组差异无统计学意义(P>0.05),两组术后排尿情况较术前均改善,差异有统计学意义(P<0.05),两组均未发生包膜穿孔、大出血、永久性尿失禁等并发症。结论低功率GreenLEP与高功率PVP均是治疗前列腺增生的有效方法,低功率GreenLEP在彻底切除腺体、减少复发、保留组织标本等方面具有一定优势。Objective To Compare therapeutic effects and safety of low power(60W)green laser enucleation of the prostate versus high power(160W)photoselective vaporization of the prostate for treatment of benign prostatic hyperplasia.Methods The clinical data of 110 patients treated with green laser for prostatic hyperplasia(BPH)in our hospital from January 2021 to December 2022 were analyzed.The patients were divided into two groups,55 cases in each group were treated with GreenLEP and PVP,respectively.The indexes of operation time,hemoglobin loss at 2 hour postoperative,postoperative hospitalization time,postoperative catheter indwelling time,urination at 3 months after surgery and surgical complications were compared between the two groups.Results Both groups of patients successfully completed the operation.Compared with the PVP group,the GreenLEP group had shorter of operation time[(47.3±19.4)min VS.(52.5±22.4)min],indwelling catheter time[(3.4±0.5)d VS.(4.6±0.4)d],postoperative hospitalization time[(4.4±0.5)d VS.(5.4±1.0)d],hemoglobin loss at 2 hour postoperative[(8.5±3.2)g/L VS.(7.8±1.9)g/L],temporary incontinence(3 VS.2),(P>0.05),Urinary symptoms were significantly improved in both groups(P<0.05),No complications such as prostatic capsule perforation,massive hemorrhage and permanent urinary incontinence occurred during the operation.Conclusion Both low-power GreenLEP and high-power PVP are effective methods for the treatment of prostate hyperplasia.Low-power GreenLEP has certain advantages in the complete removal of glands,reducing recurrence,and preserving tissue specimens.
