非伤寒沙门菌特征分析

摘  要:目的分析湖州市感染性腹泻患者非伤寒沙门菌(NTS)的血清型、药敏、同源性等特征。方法收集湖州市吴兴区人民医院和湖州市第一人民医院2020-2023年感染性腹泻患者粪便检出的NTS,并对其进行血清分型、抗菌药物敏感性试验、全基因组测序分析。结果共收集到109株NTS,以5岁以下儿童患者检出最多;检出月份以8、9月份最高;血清型以鼠伤寒沙门菌最多(42.2%);对头孢曲松的和左氧氟沙星的耐药率分别为24.8%和12.8%,多重耐药率为49.5%,检出最多的耐药谱为氨苄西林(AMP)-氨苄西林/舒巴坦(AMC)-复方新诺明(SXT),并检出1株对碳青霉烯酶耐药的菌株。ST型以ST34最多,ST34、ST19和ST11在系统发育树中形成了较大的聚类。结论湖州市NTS呈现多样化,以<5岁儿童感染居多,发病高峰期在每年的夏季,流行菌株以鼠伤寒沙门菌对应基因型为ST34型NTS为主,多重耐药率高,不同ST型遗传距离较远,相同ST型菌株则差距较小,临床治疗应注意抗菌药物的选择。Objective To understand the characteristics of serotypes,drug resistance,and homology of non-typhoidal Salmonella isolated from patients with infectious diarrhea in Huzhou City.Methods Non-typhoidal Salmonella strains were collected from stool samples of patients with infectious diarrhea at Wuxing District People's Hospital and Huzhou First People's Hospital between 2020 and 2023.These strains were subjected to serotyping,antimicrobial susceptibility testing,and whole-genome sequencing analysis.Results A total of 109 non-typhoidal Salmonella strains were collected,with the highest detection rate among children under 5 years old.The peak detection months were August and September.The most common serotype was Salmonella Typhimurium(42.2%).The resistance rates to ceftriaxone and levofloxacin were 24.8%and 12.8%,respectively,with a multidrug resistance rate of 49.5%.The most common resistance profile was AMP-AMC-SXT,and one strain resistant to carbapenemase was detected.The most prevalent sequence type(ST)was ST34,with ST34,ST19,and ST11 forming large clusters in the phylogenetic tree.Conclusion Non-typhoidal Salmonella in Huzhou City exhibits diversity,with a higher prevalence among children under 5 years old and a peak incidence during the summer months.The predominant epidemic strain is Salmonella Typhimurium with the ST34 genotype.The multidrug resistance rate is high,and genetic distances between different ST types are significant,while strains of the same ST type show smaller differences.Careful selection of antimicrobial agents is crucial in clinical treatment.
