足月单胎妊娠孕妇引产失败的影响因素分析

摘　　要：目的探讨足月单胎妊娠孕妇引产失败的影响因素。方法回顾性分析2023年1~12月实施引产的375例孕妇临床资料,根据引产结局分为引产成功组(n=77)和引产失败组(n=298)。通过单因素及多因素Logistic回归分析影响足月单胎妊娠孕妇引产结局的相关因素。结果失败组高龄初产妇、孕前BMI≥28 kg/m^(2)、孕期增重超标、新生儿体质量≥3500 g、孕周<40周、无阴道分娩史、胎儿窘迫、引产至临产时间≥24 h占比高于成功组(P<0.05);失败组身高>165 cm、胎膜早破、引产前宫颈Bishop评分低于成功组(P<0.05);多因素Logistic回归显示:高龄初产妇(OR=2.783)、孕前BMI≥28 kg/m^(2)(OR=3.596)、新生儿体质量≥3500 g(OR=3.292)、胎儿窘迫(OR=5.140)、引产至临产时间≥24 h(OR=6.824)是引产失败危险因素,引产前宫颈Bishop评分高(OR=0.419)、阴道分娩史(OR=0.387)、身高>165 cm(OR=0.255)是引产失败的保护因素(P<0.05)。结论足月单胎妊娠孕妇引产失败与高龄初产、孕前肥胖、巨大儿、胎儿窘迫、Bishop评分、阴道分娩史、引产时长及孕妇身高等有关。Objective To explore the influencing factors of induced abortion failure in full-term single pregnancy pregnant women,and provide reference for reducing cesarean section rate and improving delivery safety in clinical practice.Methods A retrospective analysis was conducted on the clinical data of 375 single live full-term pregnant women who underwent induced abortion at Hangzhou Obstetrics and Gynecology Hospital from January to December 2023.According to the outcome of induced abortion,they were divided into a successful induction group(n=77)and a failed induction group(n=298).Screening the influencing factors of induced abortion failure in full-term single pregnancy pregnant women through univariate and multivariate Logistic regression models.Results Advanced primipara,pre-pregnancy body mass index(BMI)≥28 kg/m^(2),pregnancy gain,neonatal weight≥3500,gestational age<40 weeks,no history of vaginal delivery,fetal distress,induction to labor 24 h of the proportion in failure group were significantly higher than the successful group(P<0.05).Height>165 cm,premature rupture of fetal membranes(PROM),and preinduction cervical Bishop score in failure group was lower than the successful group(P<0.05).Multivariate Logistic regression showed that older primiparous women(OR=2.783),pre pregnancy BMI≥28 kg/m^(2)(OR=3.596),newborn weight≥3500 g(OR=3.292),fetal distress(OR=5.140),and induction to labor time≥24 hours(OR=6.824)were risk factors for induced labor failure.High cervical Bishop score(OR=0.419),history of vaginal delivery(OR=0.387),and height>165 cm(OR=0.255)were protective factors for induced labor failure.Conclusion The failure of induction of labor in full-term single pregnancy pregnant women is related to advanced age of first delivery,pre pregnancy obesity,macrosomia,fetal distress,Bishop score,history of vaginal delivery,height,etc.
