摘 要:目的分析肺炎支原体肺炎(MPP)患儿血清细胞因子变化,探讨其与MPP严重程度可能存在的关联及潜在机制。方法回顾性分析2023年6~12月63例MPP患儿的临床资料,依据临床表现及检验、检查结果,将患儿分为轻症组(37例)和重症组(26例),检测并比较两组患儿血清白介素-2(IL-2)、白介素-4(IL-4)、白介素-6(IL-6)、白介素-10(IL-10)、白介素-17A(IL-17A)、γ干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)水平,通过Logistic回归及受试者操作特征(ROC)曲线进一步分析患儿血清中上述细胞因子水平对重症MPP的早期预测价值。结果重症组血清IL-2、IL-4、IL-6、IL-10、IL-17A、IFN-γ、TNF-α水平高于轻症组(P<0.05);Logistic回归分析显示,IL-6、IL-10、IFN-γ为儿童重症MPP的影响因素;ROC曲线分析显示,IL-6、IL-10、IFN-γ单独及联合诊断儿童重症MPP的曲线下面积(AUC)分别为0.80、0.73、0.74和0.93。结论IL-6、IL-10、IFN-γ联合预测儿童重症MPP有一定价值。Objective To analyze the changes in serum cytokine levels in children with Mycoplasma pneumoniae pneumonia(MPP)and explore their potential association with the severity of MPP and underlying mechanisms.Methods A retrospective analysis was conducted on the clinical data of 63 children with MPP from June 2023 to December 2023.Based on clinical manifestations,laboratory tests,and examination results,the children were divided into a mild group(37 cases)and a severe group(26 cases).The serum levels of interleukin-2(IL-2),interleukin-4(IL-4),interleukin-6(IL-6),interleukin-10(IL-10),interleukin-17A(IL-17A),interferon-gamma(IFN-γ),and tumor necrosis factor-alpha(TNF-α)were measured and compared between the two groups.Logistic regression and receiver operating characteristic(ROC)curve analyses were further performed to evaluate the early predictive value of these cytokine levels for severe MPP.Results The serum levels of IL-2,IL-4,IL-6,IL-10,IL-17A,IFN-γ,and TNF-αwere significantly higher in the severe group compared to the mild group(P<0.05).Logistic regression analysis revealed that IL-6,IL-10,and IFN-γwere influencing factors for severe MPP in children.ROC curve analysis showed that the area under the curve(AUC)for IL-6,IL-10,and IFN-γlevels individually and in combination for diagnosing severe MPP in children were 0.80,0.73,0.74,and 0.93,respectively.Conclusion The combination of IL-6,IL-10,and IFN-γhas certain value in predicting severe MPP in children.
