摘　　要：目的:观察闭合复位掌骨板外固定治疗第4、5掌骨干骨折的临床疗效。方法:自2021年1月至2023年7月,我们对29例38处第4、5掌骨干骨折患者采用闭合复位掌骨板外固定方法进行治疗。复位后观察复位情况、愈合时间及功能疗效。结果:20例骨折达到解剖对位,7例接近解剖对位,2例骨折功能对位。复位后所有患者获得随访,时间为3~6个月,平均(3.4±0.7)个月。复位后27例患者无明显移位直至骨折愈合,2例骨折发生明显移位,经再次复位固定后未发生再移位,骨折愈合时间为(7.2±0.9)周,按总主动活动度方法进行功能评定:优20例,良9例。结论:闭合复位掌骨板外固定治疗第4、5掌骨干骨折,操作简单,固定可靠,可早期功能锻炼,减少肌腱粘连和关节僵硬,手指功能恢复好。Objective:To observe the clinical efficacy of closed reduction and external fixation of metacarpal plate in the treatment of the 4th and 5th metacarpal shaft fractures.Methods:From January 2021 to July 2023,29 patients with 38 fractures of the 4th and 5th metacarpal shaft fractures were treated by closed reduction and external fixation of metacarpal plate.The reduction,healing time and functional effect were observed after operation.Results:Twenty cases of fractures reached anatomical alignment,7 cases were close to anatomical alignment,and 2 cases were functionally aligned.After resetting,all patients were follow-up for 3 to 6 months,with an average of(3.4±0.7)months.After reduction,27 patients showed no significant displacement until the fracture healed,while 2 patients experienced significant displacement.After re-reduction and fixation,no further displacement occurred.The fracture healing time was(7.2±0.9)weeks.The functional evaluation was performed using the total active range of motion method,with 20 cases rated as excellent and 9 cases rated as good.Conclusion:The application of closed reduction and external fixation of the metacarpal bone plate is a simple and reliable treatment for the 4th and 5th metacarpal shaft fractures.It can provide early functional exercise,reduce tendon adhesion and joint stiffness,and improve finger function recovery.
