祛痰化瘀中药复方对心房颤动大鼠心肌细胞凋亡及相关蛋白Bcl-2、Bax、Caspase-3和心肌纤维化的影响

摘　　要：目的探讨祛痰化瘀中药复方对心房颤动(atrial fibrillation,AF)大鼠心肌细胞凋亡及相关蛋白B淋巴细胞瘤-2(B-cell lymphoma-2,Bcl-2)、Bcl-2相关X蛋白(Bcl-2 associated X,Bax)、半胱氨酸天冬氨酸蛋白酶-3(cysteinyl aspartate specific proteinase 3,Caspase-3)和心房肌纤维化的影响。方法将90只雄性SPF级SD大鼠随机分为正常组15只和造模组75只,造模组连续7天尾静脉注射乙酰胆碱—氯化钙建立AF模型。将造模成功的大鼠随机分为模型组、胺碘酮组及中药复方低、中、高剂量组各15只。正常组和模型组采用等体积生理盐水灌胃,中药复方低、中、高剂量组大鼠每天分别予以10.31、20.63、41.25 g/kg祛痰化瘀中药复方溶液灌胃,胺碘酮组每天灌胃胺碘酮溶液25 mg/kg。用药干预两周之后,心电图记录各组大鼠AF持续时间,TUNEL染色检测心肌细胞凋亡情况,苏木素—伊红(hematoxylin-eosin staining,HE)染色观察心肌结构变化,Masson染色评价各组大鼠心肌纤维化程度并计算胶原容积分数(collagenvolume fraction,cVF),Western blot法检测各组大鼠心肌细胞凋亡相关蛋白Bcl-2、Bax、Caspase-3和I型胶原(collagentypeⅠ,ColⅠ)、Ⅲ型胶原(collagentypeⅢ,ColⅢ)的表达情况。结果(1)与正常组比较,模型组均为典型AF心电图(P<0.01);与模型组比较,中药复方中、高剂量组AF持续时间明显降低(P<0.05);(2)与正常组比较,模型组心肌组织发生纤维样改变;与模型组比较,中药复方中、高剂量组和胺碘酮组纤维样改变减少;(3)与正常组比较,模型组TUNEL阳性细胞率显著升高(P<0.01),与模型组比较,中药复方中、高剂量组及胺碘酮组TUNEL阳性细胞率显著下降(P<0.01);(4)与正常组比较,模型组Bax、Caspase-3、ColⅠ和ColⅢ蛋白表达显著升高(P<0.01),Bcl-2蛋白表达显著降低(P<0.01);与模型组比较,中药复方各剂量组及胺碘酮组Bax、Caspase-3、ColⅠ和ColⅢ蛋白表达显著降低(P<0.01),Bcl-2蛋白表Objective To explore the effects of Chinese herbs for dispelling phlegm and removing blood stasis on cardiomyocyte apoptosis and related proteins B-cell lymphoma-2(Bcl-2),Bcl-2 associated X(Bax),Cysteinyl aspartate specific proteinase 3(Caspase-3)and atrial fibrosis in rats with atrial fibrillation(AF).Methods 90 male SPF SD rats were randomly divided into the normal group(n=15)and the model group(n=75).The model was established by injecting acetylcholine-calcium chlorideinto tail vein for 7 days,and the model group was randomly divided into the model group(n=15),the amiodarone group(n=15),the low dose group(n=15),the middle dose group(n=15)and the high dose group(n=15).After successful establishment of atrial fibrillation model,rats in the control group and model group were perfused with the same volume of normal saline.The rats in the low,middle and high dose groups were given 10.31 g/kg,20.63 g/kg,41.25 g/kg compound solution of dispelling phlegm and removing blood stasis by intragastric administration of 3 mL,respectively,and amiodarone group was treated with amiodarone solution 3 mL every day,after two weeks of intervention.The duration of AF was recorded by electrocardiogram,myocardial apoptosis was detected by TUNEL staining,myocardial structural were observed by hematoxylin-eosin staining(HE),the degree of myocardial fibrosis was evaluated by Masson staining and collagen volume fraction(CVF)was calculated.The expression of cardiomyocyte apoptosis-related proteins Bcl-2,Bax,Caspase-3 and collagentypeⅠ(ColⅠ),collagentypeⅢ(ColⅢ)were detected by Western blot.Results Compared with the normal group,the typical AF ECG in the model group was significantly(P<0.01).The duration of AF in the middle and high dose groups of traditional Chinese medicine compound prescription was significantly lower than that in the model group(P<0.05).Fibrous changes occurred in myocardial tissue in the model group.Compared with the model group,the fiber-like changes was decreased in the middle and the high dose groups and the
