摘 要:目的:研究不同滤器后钙离子(Ca^(2+))浓度对连续性肾脏替代治疗(CRRT)枸橼酸抗凝的有效性。方法:于2021年5月—2023年5月本院收治的104例CRRT患者为研究对象,均给予枸橼酸抗凝,依据随机法分组,对照组(n=52)滤器后Ca^(2+)浓度为0.36~0.50mmol/L,观察组(n=52)滤器后Ca^(2+)浓度为0.20~0.35 mmol/L,比较两组生化指标、滤器、静脉管路使用寿命及CRRT期间不良反应率。结果:两组治疗后血肌酐、活化部分凝血活酶时间、凝血酶时间、尿素氮水平比较,差异无统计学意义(P>0.05)。观察组滤器使用寿命、静脉管路使用寿命长于对照组(P<0.05)。观察组CRRT期间不良反应率低于对照组(P<0.05)。结论:维持较低的滤器后Ca^(2+)浓度不会对CRRT枸橼酸抗凝效果产生影响,但可以延长滤器使用寿命,且能降低不良反应率。Objective:To study the effectiveness of calcium ion(Ca^(2+))concentration after different filters on citrate anticoagulation in continuous renal replacement therapy(CRRT).Methods:A lot of 104 patients with CRRT admitted to our hospital from May 2021 to May 2023 were studied.All patients were given citrate anticoagulation.They were divided into two groups according to the random method.The Ca^(2+)concentration after the filter in the control group(n=52)was 0.36-0.50 mmol/L,and the Ca^(2+)concentration after the filter in the observation group(n=52)was 0.20-0.35 mmol/L.The biochemical indexes,filter,venous pipeline service life and adverse reaction rate during CRRT were compared between the two groups.Results:There were no difference in serum creatinine,activated partial thromboplastin time,thrombin time and urea nitrogen levels between the two groups after treatment(P>0.05).The service life of filter and the service life of venous pipeline in the observation group were longer than those in the control group(P<0.05).The adverse reaction rate during CRRT in the observation group was lower than that in the control group(P<0.05).Conclusion:Maintaining a low filter Ca^(2+)concentration affect the filter does not affect the anticoagulant effect of CRRT citrate,but it can prolong the service life of the filter and reduce the adverse reaction rate.
