低分子肝素钙联合双嘧达莫治疗川崎病患儿的效果及对血液流变学指标及血清炎症因子水平的影响

摘　　要：目的:探讨低分子肝素钙联合双嘧达莫对川崎病患(KD)儿血液流变学指标及血清炎症因子水平的影响。方法:回顾性分析2021年2月—2024年2月我院收治的83例KD患儿临床资料,以不同治疗方法分为单药组(n=43)、联合组(n=40)。单药组采用双嘧达莫治疗,联合组采用低分子肝素钙联合双嘧达莫治疗,比较两组临床疗效、症状消退时间、血液流变学、血清炎症指标、不良反应总发生率。结果:联合组总有效率为92.50%,高于单药组的74.42%(P<0.05);联合组淋巴结肿胀、多形性皮疹、结膜充血、手足硬性水肿、发热消退时间短于单药组(P<0.05);联合组治疗后全血高切黏度、血浆黏度、红细胞聚集指数、全血低切黏度、PCT、IL-6、IL-1、HMGB1低于单药组(P<0.05);联合组不良反应总发生率与单药组比较,差异无统计学意义(P>0.05)。结论:低分子肝素钙联合双嘧达莫治疗KD患儿的效果显著,可有效减轻患儿症状,改善血液流变学,降低机体炎症反应,且具有安全性。Objective:To investigate the effects of low molecular weight heparin calcium combined with dipyridamole on hemorheological indexes and serum inflammatory factors in children with Kawasaki disease(KD).Methods:The clinical data of 83 children with KD admitted to our hospital from February 2021to February 2024 were retrospectively analyzed.According to different treatment methods,they were divided into single drug group(n=43)and combined group(n=40).The single drug group was treated with dipyridamole,and the combined group was treated with low molecular weight heparin calcium combined with dipyridamole.The clinical efficacy,symptom regression time,hemorheology,serum inflammation index and total incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the combined group was 92.50%,which was higher than 74.42%of the single drug group(P<0.05).The regression time of lymph node swelling,polymorphic rash,conjunctival congestion,hand-foot hard edema and fever in the combined group was shorter than that in the single drug group(P<0.05).After treatment,the whole blood high shear viscosity,plasma viscosity,erythrocyte aggregation index,whole blood low shear viscosity,PCT,IL-6,IL-1 and HMGB1 in the combined group were lower than those in the single drug group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the combined group and the single drug group(P>0.05).Conclusion:Low molecular weight heparin calcium combined with dipyridamole in the treatment of children with KD has a significant effect,which can effectively reduce the symptoms of children,improve hemorheology,reduce the body's inflammatory response,and is safe.
