摘　　要：目的:研究西维来司他钠以及气道压力释放通气两种疗法联用对急性呼吸窘迫综合征的效果。方法:选择2021年6月—2023年6月徐州医科大学第二附属医院/徐州矿务集团总医院的50例急性呼吸窘迫综合征患者,随机分为两组,每组25例。对照组单用气道压力释放通气疗法,观察组在对照组基础上联用西维来司他钠。比较两组血气指标[活性碳酸酐酶(CA)、氧合指数(PaO_(2)/FiO_(2))、血乳酸(Lac)]、肺功能[预测性呼气末正压(PEEP)、肺动态顺应性(Cdyn)、气道阻力(Raw)]、炎症因子[血乳酸(LPS)、降钙素原(PCT)和溶酶体酶(LE)]。结果:观察组总有效率为96.00%,高于对照组的76.00%(P<0.05);治疗后,观察组Lac、Raw、PEEP、LPS、PCT和LE水平低于对照组,CA、PaO_(2)/FiO_(2)、Cdyn水平高于对照组(P<0.05)。结论:西维来司他钠以及气道压力释放通气疗法联用能有效改善肺功能和血气指标,减轻急性呼吸窘迫综合征患者的炎症反应,效果显著。Objective:To study the effect of sivelestat sodium combined with airway pressure release ventilation on acute respiratory distress syndrome.Methods:From June 2021 to June 2023,50 patients with acute respiratory distress syndrome in the Second Affiliated Hospital of Xuzhou Medical University/General Hospital of Xuzhou Mining Group were randomly divided into two groups,with 25 patients in each group.The control group was treated with airway pressure release ventilation alone,and the observation group was treated with sivelestat sodium on the basis of the control group.Blood gas indexes[active carbonic anhydrase(CA),oxygenation index(PaO_(2)/FiO_(2)),blood lactic acid(Lac)],lung function[predictive positive end-expiratory pressure(PEEP),lung dynamic compliance(Cdyn),airway resistance(Raw)],inflammatory factors[blood lactic acid(LPS),procalcitonin(PCT)and lysosomal enzyme(LE)]were compared between the two groups.Results:The total effective rate of the observation group was 96.00%,which was higher than 76.00%of the control group(P<0.05).After treatment,the levels of Lac,Raw,PEEP,LPS,PCT and LE in the observation group were lower than those in the control group,and the levels of CA,PaO_(2)/FiO_(2)and Cdyn were higher than those in the control group(P<0.05).Conclusion:The combination of sivelestat sodium and airway pressure release ventilation therapy can effectively improve lung function and blood gas indexes,and reduce the inflammatory response of patients with acute respiratory distress syndrome.
