MRI平扫与动态增强成像对乳腺良恶性病变的诊断价值对比  

Comparison of Diagnostic Value between MRI Plain Scan and Dynamic Contrast-enhanced Imaging for Benign and Malignant Breast Lesions

摘　　要：分析磁共振(MRI)平扫与动态增强成像(DCE-MRI)对乳腺良恶性病变的诊断价值。选择西北妇女儿童医院2023年1月―2023年12月期间收治的1000例乳腺病变患者作为研究对象,全部患者均接受MRI平扫与动态增强成像检查,分析不同诊断方式的检出率、诊断效能,同时分析良恶性病变患者的DCE-MRI参数。结果显示,病理检查发现良性肿瘤患者673例,恶性肿瘤患者327例,DCE-MRI检出率显著高于MRI平扫(P<0.05);DCE-MRI检出准确性、灵敏度、特异度均显著高于MRI平扫,误诊及漏诊率显著低于MRI平扫(P<0.05);良性病变组增强后病灶达峰时间(TTP)水平显著高于恶性病变组,增强后病灶内造影剂最大浓度(MAX Conc)、容积运转常数(K^(trans))、速率常数(K_(ep))、血管外细胞外间隙容积分数(V_(e))水平显著低于恶性病变组(P<0.05)。研究发现,在乳腺良恶性病变的诊断中,MRI为重要检查技术,其中DCE-MRI检查效果优于MRI平扫,能够为临床诊疗提供可靠参考。To analyze the diagnostic value of magnetic resonance imaging(MRI)plain scan and dynamic contrast enhanced MRI(DCE-MRI)for benign and malignant breast lesions.1000 patients with breast lesions admitted to Northwest Women's and Children's Hospital from January 2023 to December 2023 were selected as the research subjects.All patients underwent MRI plain scan and dynamic enhanced imaging examination,and the detection rate and diagnostic efficacy of different diagnostic methods were analyzed.At the same time,the DCE-MRI parameters of patients with benign and malignant lesions were analyzed.The results showed that pathological examination revealed 673 patients with benign tumors and 327 patients with malignant tumors.The detection rate of DCE-MRI was significantly higher than that of MRI plain scan(P<0.05);The accuracy,sensitivity,and specificity of DCE-MRI detection were significantly higher than those of MRI plain scan,and the misdiagnosis and missed diagnosis rates were significantly lower than those of MRI plain scan(P<0.05);The time to peak(TTP)level of enhanced lesions in the benign lesion group was significantly higher than that in the malignant lesion group.The maximum concentration of contrast agent(MAX Conc),volume transfer constant(K^(trans)),rate constant(K_(ep)),and extracellular space fraction(V_(e))levels in the enhanced lesions were significantly lower than those in the malignant lesion group(P<0.05).Research has found that MRI is an important diagnostic technique in the diagnosis of benign and malignant breast lesions,with DCE-MRI being more effective than MRI plain scan and providing reliable reference for clinical diagnosis and treatment.
