重复经颅磁刺激联用语言康复训练治疗脑卒中后言语功能障碍患者的临床观察

摘　　要：观察重复经颅磁刺激(rTMS)联用语言康复训练治疗脑卒中后言语功能障碍患者的临床应用效果。选取2023年1月―2024年1月大连医科大学附属第二医院收治的74例脑卒中后言语功能障碍患者进行研究,采用随机数字表法将其分为观察组和对照组,每组37例。对照组采用常规语言康复训练治疗,观察组采用rTMS联合语言康复训练治疗,并对比两组患者的临床治疗效果。结果显示,治疗8周后,两组中国康复研究中心汉语标准失语症检查量表(CRRCAE)评分和失语商(AQ)分值显著高于治疗前,且观察组明显高于对照组(P<0.05);治疗8周后,观察组血氧水平依赖功能磁共振成像(BOLD-fMRI)检测显示大脑右侧半球激活区较左侧明显缩小(P<0.05)。研究发现,将rTMS与系统化的语言康复训练相结合,能够显著提升脑卒中后出现言语功能障碍患者的语言能力。To observe the clinical application effect of repeated transcranial magnetic stimulation(rTMS)combined with language rehabilitation training in the treatment of post-stroke speech dysfunction patients.74 patients with post-stroke speech dysfunction admitted to the Second Affiliated Hospital of Dalian Medical University from January 2023 to January 2024 were selected for the study.They were randomly divided into an observation group and a control group using a random number table method,with 37 patients in each group.The control group received conventional language rehabilitation training treatment,while the observation group received rTMS combined with language rehabilitation training treatment,and the clinical treatment effects of the two groups of patients were compared.The results showed that after 8 weeks of treatment,the scores of CRRCAE and AQ in both groups were significantly higher than before treatment,and the observation group was significantly higher than the control group(P<0.05);After 8 weeks of treatment,blood oxygen level dependent functional magnetic resonance imaging(BOLD fMRI)in the observation group showed a significant reduction in the activated area of the right hemisphere of the brain compared to the left(P<0.05).Research has found that combining rTMS with systematic language rehabilitation training can significantly improve the language ability of patients with speech dysfunction after stroke.
