机械性血栓清除联合置管溶栓治疗静脉血栓栓塞症的疗效分析

摘　　要：探究机械性血栓清除联合置管溶栓治疗静脉血栓栓塞症(VTE)的疗效。回顾性分析广西崇左市人民医院2021年10月―2023年10月期间收治的93例VTE患者的临床资料,所有患者均接受机械性血栓清除联合置管溶栓治疗。分析血栓清除率、手术并发症情况、静脉通畅率及下肢肿胀改善情况。结果显示,治疗后,血栓清除优良率为95.7%。93例VTE患者中,2例穿刺部位血肿经压迫吸收;22例使用AngioJet(血栓抽吸系统)后出现短暂血尿,24小时内消失;10例肺动脉血栓清除中出现短暂胸闷和心律失常,暂停后缓解。8例髂总静脉狭窄患者球囊扩张效果不佳,行支架植入后1个月血管通畅率为(83.42±20.95)%。治疗后,患者健侧与患侧大腿及小腿的周径差均显著低于治疗前(P<0.05)。研究发现,机械性血栓清除联合置管溶栓治疗VTE具有较高的血栓清除率和良好的临床疗效,能够显著改善患者的下肢肿胀症状和静脉通畅情况。To explore the efficacy of mechanical thrombus clearance combined with catheter thrombolysis in the treatment of venous thromboembolism(VTE).Retrospective analysis of clinical data of 93 VTE patients admitted to The People's Hospital of Chongzuo from October 2021 to October 2023.All patients received mechanical thrombus clearance combined with catheter thrombolysis treatment.Analyze thrombus clearance rate,surgical complications,venous patency rate,and improvement of lower limb swelling.The results showed that after treatment,the excellent rate of thrombus clearance was 95.7%.Among 93 VTE patients,2 cases had hematoma absorbed by compression at the puncture site;22 cases experienced transient hematuria after using AngioJet(thrombus aspiration system),which disappeared within 24 hours;Transient chest tightness and arrhythmia occurred during the clearance of pulmonary artery thrombosis in 10 cases,which were relieved after a pause.Eight patients with common iliac vein stenosis had poor balloon dilation results,and the vascular patency rate was(83.42±20.95)%one month after stent implantation.After treatment,the difference in circumference between the healthy and affected thighs and calves of the patient was significantly lower than before treatment(P<0.05).Research has found that mechanical thrombus clearance combined with catheter thrombolysis for VTE has a high thrombus clearance rate and good clinical efficacy,which can significantly improve patients'lower limb swelling symptoms and venous patency.
