老年抑郁症状患者BDNF基因多态性与甲状腺激素水平的相关性分析

摘  要:目的探讨在老年人抑郁症(MDD)发生中,脑源性神经营养因子(BDNF)基因多态性与甲状腺激素水平的作用及相关性。方法选择在2022年8月到2024年3月之间在宁波大学附属康宁医院和象山县中医医院医疗健康集团三院院区进行治疗的的老年抑郁症患者104名纳入实验组,同期健康查体的志愿者102例纳入对照组。进行rs10835210、rs6265的单核苷酸多态性(SNP)测定,并采用Hardy-Weinberg遗传平衡方法进行检验。检测所有研究对象BDNF基因rs10835210、rs6265两个关键位点多态性和甲状腺激素水平,比较BDNF基因多态性与MDD患病的关联,以及与甲状腺素(T_(4))、促甲状腺激素(TSH)、三碘甲腺原氨酸(T_(3))之间的关联。比较实验组和对照组的临床资料,分析MDD患病的影响因素,探讨相关影响因素与不同基因型对MDD患病的交互作用。结果将MDD实验组与对照组的2个SNP位点的基因型和等位基因进行HardyWeinberg遗传平衡可知,P均>0.05,符合样本遗传平衡规律。与对照组相比,实验组患者的TSH增加,T_(3)、T_(4)降低,差异均有统计学意义(P<0.05)。rs6265位点和rs10835210位点与MDD患病风险在等位基因模型、加性模型、显性模型和隐性模型中均具有明显关联(P<0.05)。在实验组中,rs6265和rs10835210与TSH、T_(3)和T_(4)在加性模型和显性模型中有明显关联(P<0.05)。多因素logistic回归分析显示,TSH、T_(3)、T_(4)、rs6265、rs10835210均是影响患者MDD的独立影响因素(P<0.05)。交互作用分析调整混杂因素后,结果显示rs6265基因型和rs10835210基因型与TSH、T_(3)、T_(4)对MDD发生的影响均存在交互作用。结论rs6265和rs10835210与MDD患病在等位基因模型、加性模型、显性模型和隐性模型中均有明显关联,年龄、独居、家族史遗传情况、TSH、T_(3)、T_(4)、rs6265、rs10835210等因素均是患者MDD的独立影响因素,据此建立MDD发病的风险预测模型稳定且预测效能良好,Objective To understand the roles of brain-derived neurotrophic factor(BDNF)gene polymorphisms and thyroid hormone levels in the occurrence of major depressive disorder(MDD)among the elderly.Methods A total of 104 elderly patients with depression who were treated in two hospitals from August 2022 to March 2024were included in the experimental group,and 102 healthy volunteers were included in the control group.The single nucleotide polymorphisms(SNPS)of rs10835210 and rs6265 were determined,and the Hardy-Weinberg genetic equilibrium method was used to test.BDNF gene rs10835210 and rs6265 key loci polymorphisms and thyroid hormone levels were detected in all subjects.The relationships between BDNF gene polymorphism and MDD,as well as thyroxine(T_(4)),thyroid stimulating hormone(TSH)and triiodothyronine(T_(3))were compared.The clinical data of the experimental group and the control group were compared,the influencing factors of MDD were analyzed,and the interaction between related influencing factors and different genotypes on MDD was analyzed.Results The Hardy-Weinberg genetic balance of the genotypes and alleles of the two SNPS between the MDD experimental group and the control group showed that all P values were>0.05,which was in line with the law of genetic balance of the samples.Compared with the control group,the TSH in the experimental group significantly increased(P<0.05),and T_(3)and T_(4)significantly decreased(P<0.05).The rs6265 and rs10835210 loci were significantly associated with the risk of MDD in the allele model,additive model,dominant model and recessive model(P<0.05).In the experimental group,rs6265 and rs10835210 were significantly associated with TSH,T_(3)and T_(4)in both additive and dominant models(P<0.05).Multivariate Logistic regression analysis showed that TSH,T_(3),T_(4),rs6265 and rs10835210 were independent influencing factors for MDD(P<0.05).After adjusting the confounding factors by interaction analysis,the results showed that there were interactions between rs6265 and rs10835210 genot
