摘　　要：目的对比研究开腹卵巢囊肿剥除术(OOCD)与腹腔镜卵巢囊肿剥除术(LOCD)对卵巢囊肿(OC)患者卵巢储备功能的影响。方法队列研究。抽取2018年9月至2021年1月商丘市第一人民医院收治的OC患者92例,按随机数字表法分为OOCD组和LOCD组,每组46例。OOCD组采用OOCD治疗,LOCD组采用LOCD治疗。比较两组围术期情况、卵巢储备功能[雌二醇(E2)、促卵泡激素(FSH)、抗苗勒管激素(AMH)]、免疫功能[免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)]、并发症发生率及复发率。结果LOCD组手术时间、住院天数短于OOCD组,术中失血量少于OOCD组(P<0.05)。术后6个月,LOCD组E2、AMH水平高于OOCD组,FSH水平低于OOCD组(P<0.05)。术后6个月,LOCD组IgA、IgM、IgG水平高于OOCD组(P<0.05)。LOCD组并发症发生率(2.17%,1/46)低于OOCD组(17.39%,8/46),P<0.05。术后随访12个月,LOCD组复发率(2.17%,1/46)低于OOCD组(19.57%,9/46),P<0.05。结论与OOCD比较,LOCD治疗能缩短OC患者手术和住院时间,改善患者卵巢储备功能,并能减少术后盆腔感染等并发症情况,降低复发率。Objective To compare the effects of open ovarian cystectomy(OOCD)and laparoscopic ovarian cystectomy(LOCD)on ovarian reserve function in patients with ovarian cysts(OC).MethodsA total of 92 OC patients admitted to the First People’s Hospital of Shangqiu from September 2018 to January 2021 were selected for the cohort study,and they were divided into the OOCD group and the LOCD group according to random number table method,with 46 cases in each group.The OOCD group was treated by OOCD,and the LOCD group was treated by LOCD.The perioperative conditions,ovarian reserve function evaluated by estradiol(E2),follicle stimulating hormone(FSH)and anti-mullerian hormone(AMH),immune function evaluated by immunoglobulin A(IgA),immunoglobulin M(IgM)and immunoglobulin G(IgG),complication rate and recurrence rate were compared between the two groups.ResultsThe operation duration and hospital stay in the LOCD group were shorter than those in the OOCD group,and the intraoperative blood loss was less than that in the OOCD group(P<0.05).At 6 months after operation,the levels of E2 and AMH in the LOCD group were higher than those in the OOCD group,while the level of FSH was lower than that in the OOCD group(P<0.05).At 6 months after operation,the levels of IgA,IgM and IgG in the LOCD group were higher than those in the OOCD group(P<0.05).The incidence of complications in the LOCD group(2.17%,1/46)was lower than that in the OOCD group(17.39%,8/46),P<0.05.After 12 months of follow-up,the recurrence rate of the LOCD group(2.17%,1/46)was lower than that of the OOCD group(19.57%,9/46),P<0.05.ConclusionsCompared with OOCD,LOCD in the treatment of OC can shorten the operation duration and hospital stay,improve the ovarian reserve function of patients,and reduce the incidence of postoperative complications for example,pelvic infection,and decrease the recurrence rate.
