功能性鼻内镜手术治疗嗜酸性粒细胞型慢性鼻窦炎伴鼻息肉的临床评价

摘　　要：目的评价功能性鼻内镜手术(FESS)治疗嗜酸性粒细胞型慢性鼻窦炎(CRS)伴鼻息肉(NP)的临床效果。方法回顾性抽取2021年1月至2022年12月南阳市中心医院收治的嗜酸性粒细胞型CRS伴NP患者60例,按随机数字表法分为常规组和FESS组,每组30例。常规组采用常规手术,FESS组采用FESS。比较两组围术期指标、疼痛数字评分表(NRS)、Lund-Kennedy内镜评分、嗅觉功能评分及并发症发生率。结果FESS组术中出血量少于常规组,手术时长、鼻腔通气时间、住院天数短于常规组(P<0.05);术后6个月,FESS组NRS评分、Lund-Kennedy内镜评分低于常规组(P<0.05);术后3个月,FESS组嗅觉功能评分低于常规组(P<0.05);FESS组并发症发生率(3.33%,1/30)低于常规组(26.67%,8/30),P<0.05。结论FESS用于嗜酸性粒细胞型CRS伴NP患者中可缩短手术时长,减少出血量,减轻机体疼痛,促使患者鼻腔通气,改善其嗅觉功能,且并发症较少,安全性良好。Objective To evaluate the clinical effect of functional endoscopic sinus surgery(FESS)in the treatment of eosinophilic chronic rhinosinusitis(CRS)with nasal polyps(NP).MethodsA total of 60 patients with eosinophilic CRS complicated by NP treated in Nanyang Central Hospital from January 2021 to December 2022 were selected retrospectively.And they were divided into the conventional group and the FESS group according to random number table method,with 30 cases in each group.The conventional group was treated by conventional operation,while the FESS group was treated by FESS.Perioperative indexes,pain numerical rating scale(NRS),Lund-Kennedy endoscopic score,olfactory function score and complication rate were compared between the two groups.ResultsCompared with the conventional group,the FESS group had less intraoperative blood loss,shorter operation duration,shorter nasal ventilation time and shorter hospital stay(P<0.05).Six months after operation,the NRS score and Lund-Kennedy endoscopic score in the FESS group were lower than those in the conventional group(P<0.05).Three months after operation,the score of olfactory function in the FESS group was lower than that in the conventional group(P<0.05).The incidence of complications in the FESS group(3.33%,1/30)was lower than that in the conventional group(26.67%,8/30),P<0.05.ConclusionsFESS in the treatment of eosinophilic CRS with NP can shorten operation duration,reduce blood loss,relieve body pain,promote nasal ventilation,and improve the olfactory function of patients,with fewer complications and good safety.
