摘　　要：目的探讨动态心电图诊断脑卒中患者心房颤动的价值。方法回顾性抽取2023年12月至2024年4月安康市中医医院收治的脑卒中患者87例,均行动态心电图检查,依据心房颤动发生情况将其分为对照组(41例,非心房颤动者)和观察组(46例,心房颤动者)。观察组46例中低负荷15例,中负荷18例,高负荷13例。比较两组P波最大时限(Pmax)、P波最小时限(Pmin)以及P波离散度(Pd),比较不同负荷量心房颤动患者的病情,比较心房颤动患者白天与晚上R-R间歇出现情况。结果对照组Pd、Pmax小于观察组,Pmin大于观察组(P<0.05)。低负荷心房颤动患者的心房颤动次数、心率、心房颤动持续最长时间少于中负荷患者和高负荷患者,中负荷患者上述指标低于高负荷患者(P<0.05)。46例心房颤动患者中,晚上R-R间歇发生率(82.61%,38/46)高于白天(60.87%,28/46),心室率[(70.81±3.48)次/min]低于白天[(78.44±7.61)次/min],P<0.05。结论动态心电图在卒中患者中能准确诊断心房颤动,且能评估病情,可为临床诊治提供有效指导。Objective To investigate the value of the ambulatory electrocardiography in the diagnosis of atrial fibrillation in patients with stroke.MethodsEighty-seven stroke patients admitted to Ankang Hospital of Traditional Chinese Medicine from December 2023 to April 2024 were selected retrospectively.And they all underwent ambulatory electrocardiography.The selected patients were divided into the control group(41 cases,without atrial fibrillation)and the observation group(46 cases,with atrial fibrillation)according to the occurrence of atrial fibrillation.Among the 46 cases in the observation group,15 cases were low-loaded,18 cases were moderate-loaded,and 13 cases were high-loaded.The maximum duration of P wave(Pmax),the minimum duration of P wave(Pmin)and the dispersion of P wave(Pd)were compared between the two groups.The disease conditions of atrial fibrillation patients with different cardiac loads were compared.And the occurrence of R-R interval during the day and night in patients with atrial fibrillation were compared.ResultsThe Pd and Pmax of the control group were smaller than those of the observation group,and the Pmin of the control group was greater than that of the observation group(P<0.05).The number of atrial fibrillation,the heart rate,and the longest duration of atrial fibrillation in atrial fibrillation patients with low-load were lower than those in patients with moderate-load and high-load(P<0.05);and the above indicators in patients with moderate-load were lower than those in patients with high-load(P<0.05).Among the 46 patients with atrial fibrillation,the incidence of R-R interval at night(82.61%,38/46)was higher than that during the day(60.87%,28/46),P<0.05;while the ventricular rate at night was(70.81±3.48)times/min,lower than the(78.44±7.61)times/min during the day(P<0.05).ConclusionsAmbulatory electrocardiography can accurately diagnose atrial fibrillation in stroke patients,and can evaluate the disease condition,which can provide effective guidance for clinical diagnosis and treatment.
